NRC INSPECTION REPORT N05. 50-413/90-24, 50-414/90-24
This refers to the inspection conducted by the resident inspectors on August 3,
1990 to September 5,1990.
The inspection included a review of activities .
authorized for your Catawba facility.. At the conclusion of the inspection, the
findings were discussed with those members of your staff identified in the
enclosed inspection report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.

The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violation, references to pertinent requirements, and
elements to be included in your response are presented in the enclosed Notice
of Violation.- This violation is of particular concern to the NRC since it-
involves - licensed operators
signing off safety related procedure

requirements as being complete when in fact they were not.
In your response to
this violation please devote particular attention to determining the root cause
i
.of this event and your corrective actions to preclude recurrence.
In accordance with 10CFR 2.790(a), a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosures are not subject to the
clearance procedures of the Office of Management and Budget as required by the-
Paperwork Reduction Act of 1980, Pub. L. No.96-511.
