CONFIRMATION OF MEETING ARRANGEMENTS - CATAWBA UNITS 1 AND 2 This conif rms the arrangements made between" Mr. R. L. Gill of Duke Power Company and Mr. M. B. Shymlock of my staff concerning an enforcemnt conference to be conducteu at the Catawba Nuclear Station on September 28. 1989, at 9:30 a.m. The purpose of the meeting will be to discuss two issues. The first issue is the apparent failure to take adequate corrective action in response to a failed surveillance test on the Unit 2 turbine driven auxiliary feedwater pump. The second issue regards the apparent inability of both Unit 2 motor driven auxiliary feedwater pumps to align their suctions to the Nuclear Service Water system, the assured source of makeup water. This was discovered during the performance of a piping flush procedure. The areas we would like to discuss are identified in the enclosures.

ENCLOSURE 1 TURBINE DRIVEN AUXILIARY FEEDWATER PUMP ITEMS.

1. Reasons for performing maintenance on the turbine driven CA pump outside i the requirements of the work request system.
2. Adequacy of the maintenance and operability evaluation prior to returning the turbine driven pump to an operable status after the July 31 overspeed.
3. Prudence of performing maintenance on 'B' train auxiliary feedwater support systems while concerns remained relative to turbine driven pump.
4. Root cause of the failure of the turbine.

5. Corrective action planned for the turbine and for maintenance programs.
6. Assessment of safety significance of event in question.

ENCLOSURE 2 AUXILIARY FEEDWATER SUCTION TO NUCLEAR SERVICE WATER REALIGNMENT ITEMS

1. Safety significance of having all three CA pumps inoperable.
2. The apparent lack of understanding by the operator as to the safety significance of removing the automatic capability of the CA valves.
3. The inadequacy of the flush procedure.
4. Failure of the operator to imediately initiate corrective actions when Bypass Panel indications were observed. (This is required by procedures) t -. -_. ._. ___-_:-__
