Dear Mr. Penfield:
On September 30, 2021, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Perry Nuclear Power Plant. On October 28, 2021, the NRC inspectors discussed the results of this inspection with you and other members of your staff. The results of this inspection are documented in the enclosed report.

SUMMARY
The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Perry Nuclear Power Plant, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS
The plant began the inspection period at full power. On August 28, 2021, reactor power was lowered to 65 percent to perform a rod sequence exchange. The Unit was returned to full power on August 31, 2021. The Unit remained at or near full power for the remainder of the inspection period with several exceptions. Specifically, on several occasions, power was reduced between 1 and 16 percent ranging from a few hours to several days due to environmental conditions.
INSPECTION SCOPES
Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.
Starting on March 20, 2020, in response to the National Emergency declared by the President of the United States on the public health risks of the coronavirus (COVID-19), resident and regional inspectors were directed to begin telework and to remotely access licensee information using available technology. During this time, the resident inspectors performed periodic site visits each week, increasing the amount of time on site as local COVID-19 conditions permitted.
As part of their onsite activities, resident inspectors conducted plant status activities as described in IMC 2515, Appendix D; observed risk significant activities; and completed on site portions of IPs. In addition, resident and regional baseline inspections were evaluated to determine if all or a portion of the objectives and requirements stated in the IP could be performed remotely. If the inspections could be performed remotely, they were conducted per the applicable IP. In some cases, portions of an IP were completed remotely and on site. The inspections documented below met the objectives and requirements for completion of the IP.

INSPECTION RESULTS
Minor Violation 71152 Minor Violation: On February 2, 2021, during performance of the SVI-E22-T2001, HPCS pump and valve operability test, Revision 35, the NRC inspectors noted that the licensee had left the high-pressure core spray (HPCS) pump room water-tight door open during the surveillance test for passage. Procedure OAI-0201 Operations General instructions and Good Operating Practices Revision 46, Attachment 1, section 6, states in part that, ECCS/RCIC pump room water-tight doors were designed to minimize suppression pool inventory loss due to an unisolable leak in the suction piping upstream of the containment isolation valve. These water-tight doors should remain closed except during the passage through them. Do not leave the door open or blocked open without permission from the Shift manager. The licensee restored compliance by closing the watertight door following the completion of the surveillance test.
After multiple conversations with staff and management, the inspectors concluded permission to maintain the door open for other than passage was neither sought nor given. The inspectors questioned licensee staff to understanding better if their observation was in line with OAI-0201 procedural requirements. The licensee generated an action tracking item, ATL-2021-0270-ATA-59, to answer the inspectors questions but did not address the inspectors concerns associated with the procedural implementation. The inspectors reengaged licensee staff to reemphasize the concern related to not restoring plant configuration following passage through the HPCS pump room water-tight door. The licensee generated CR 2021-05010, NRC ID: Actions of OAI-201 not strictly adhered to during testing, to document the inspectors concern.
Screening: The inspectors determined the performance deficiency was minor because they answered no to all of the questions contained in Section 05.01 of IMC 0612, Issue Screening. The performance deficiency is minor consistent with Example 4.a. of IMC 0612, Appendix E. Specifically, based on several factors such as personnel in the vicinity and the existence of control room alarms to alert operators of a leak in the room, leaving the doors open for a short duration had no safety impact and is, therefore, minor.
Enforcement:
This failure to comply with 10 CFR 50 Appendix B, Criterion V, Instructions, Procedures, and Drawings, constitutes a minor violation that is not subject to enforcement action in accordance with the NRCs Enforcement Policy.
