REPORT The attachment to this letter provides a copy of the inservice inspection summary report for activities at Donald C. Cook Nuclear Plant Unit 2, National Board (State of Michigan)

Number 1 (Mich. M-324232-M), located in Bridgman, Michigan. The plant is owned by Indiana Michigan Power Company, P.O. Box 60, Fort Wayne, IN, 46801.

The Unit 2 commercial service date is July 1, 1978, and the Authorized Code Inspectors during the report period were Mr. A. S. Jimenez and Mr. J. A. Longenberger, both of whom were employed by ABS Group, Inc. of Houston, Texas.

The examinations, tests, replacements, repairs performed, conditions observed, and corrective measures taken are summarized in the attached report. Inservice inspection activities were performed in accordance with the rules and requirements of the American Society of Mechanical Engineers' Code,Section XI, 1989 Edition (no Addenda) and 1992 Edition with the 1992 Addenda, as applicable.

INDIANA MICHIGAN POWER COMPANY DONALD C. COOK NUCLEAR PLANT, UNIT 2, BRIDGMAN, MI COMMERCIAL SERVICE DATE 7-1-1978 SECTION 1 INTRODUCTION This report summarizes the Inservice Inspection (ISI) activities performed at the Donald C. Cook Nuclear Plant (CNP) Unit 2, for the period of November 10, 2004, to May 5, 2006. This report addresses examinations and activities associated with the Unit 2 Cycle 16, 2006, Refueling Outage. This is the second and final refueling outage of the Second Inspection Period of the Third Ten-Year Interval.

These activities were performed in accordancb with the requirements of the ASME Boiler and Pressure Vessel Code,Section XI, Rules for Inservice Inspection of Nuclear Power Plant Components. As of July 1, 1996, the third ten-year inspection interval began and the applicable code of record for ISI activities became ASME Section XI, 1989 Edition (no Addenda) and 1992 Edition with 1992 Addenda for the Containment as applicable.

The purpose of this report is to satisfy the requirements of IWA-6000 for the submittal of an Inservice Inspection Summary Report at the completion of each refueling outage.

Examination and tests conducted since the preceding summary report are included in this report. Repairs and replacements are also included in this report per the requirements of the 1989 Edition of ASME Section XI, IWA-6220.

Contents of this report have been reviewed and verified correct with supporting documentation available at the plant for review if required.

COOK NUCLEAR PLANT, UNIT 2 BRIDGMAN, MI COMMERCIAL SERVICE DATE 7-1-1978 SECTION 3 ABSTRACT OF EXAMINATIONS, RESULTS, AND CORRECTIVE ACTIONS FOR CLASS 1 AND CLASS 2 COMPONENTS Inservice examinations were performed during the period November 10, 2004, to May 5, 2006. This report addresses examinations and activities associated with the Third 10-Year Interval, Second Inspection Period, -Second Refueling Outage (Unit 2 Cycle 16). All examinations were in accordance with the 1989 Edition (no Addenda) and 1992 Edition with 1992 Addenda as applicable.

Examinations were scheduled and performed according to the Third Ten-Year Long Term Inservice Examination Plan for Class 1, 2, and 3 Systems and Components for CNP, Unit 2. This is the currently approved ISI examination plan that identifies specific activities to be performed on a refueling outage basis.

Twenty-six (26) ASME Class 1 and twenty-eight (28) ASME Class 2 components were examined. A combination of Ultrasonic, Magnetic Particle, Liquid Penetrant and Visual examination methods were used. Two supports failed the VT-3 examination. Support 2-ACS-R-913 had a loose nut on an embedment plate that was evaluated as an original construction defect and not inservice induced and documented via Work Request 06016013. Support 2-AFW-C-4034 also had a loose nut on a-pipe clamp. This condition was evaluated by Design Engineering (Condition Report 06102054) and Engineering Programs and found acceptable. The cause was attributed to original construction. The supports upstream, downstream and in the immediate vicinity of these two supports were examined and found acceptable. No sample expansions were required.

To proactively address issues with Primary Water Stress Corrosion Cracking (PWSCC) of Alloy 82/182 weld locations in the Unit 2 pressurizer safe ends, all the safe ends on the Unit 2 pressurizer received Pre-emptive Weld Overlays (PWOL) to mitigate the potential for PWSCC. Each location received a fully qualified ASME Section XI Appendix VIII, Supplement 11 ultrasonic examination and a surface examination over 100% of the required weld volume. No evidence of PWSCC or other rejectable indications were observed.

During the Unit 2, Cycle 16 refueling outage, Indiana Michigan Power Company (I&M),

the licensee for CNP Unit 2, performed a bare-metal visual inspection of the CNP Unit 2 reactor pressure vessel (RPV) bottom-mounted instrumentation (BMI) nozzle penetrations, and bare-metal and volumetric examinations of the RPV upper head in response to NRC Revised Order EA-03-009, "Issuance of Order Establishing Interim 7

Inspection Requirements for Reactor Pressure Vessel Heads at Pressurized Water Reactors".

The BMI inspection was conducted by contractor and I&M personnel, and included personnel qualified as American Society of Mechanical Engineers Code,Section XI, Level II examiners. The bottom two sections of insulation were lowered to allow unobstructed access to all penetrations and the membrane areas of the lower reactor vessel. A pole-mounted camera was used to perform a VT-2 examination to VT-3 examination criteria for distance and lighting on all 58 Unit2 RPV BMI nozzle penetrations, including 360 degrees around each BMI penetration. No boric acid deposits were found at any RPV BMI nozzle penetration or anywhere on the RPV bottom-head surface area. No boric acid residue sampling or analysis was required or performed. The as-found condition of the Unit 2 RPV bottom head was identical to the as-left condition following the bottom cleaning and re-inspection performed during the Unit 2, Cycle 15 refueling outage. No indications of through-wall leakage were found during the Unit 2, Cycle 16 RPV BMI inspection. The inspection results were digitally recorded, and copies of the video files were saved to computer drives as video clips.

A 100% bare-metal visual examination of the Unit 2 RPV upper head showed no evidence of pressure boundary leakage. However, evidence of above-head leakage from an external source was discovered near Penetration Number 42. To ensure that the source of residue was not the result of pressure-boundary leakage, the following were performed.

• A chemical sample analysis, which determined the residue to be 1.3 to 1.6 years old, was performed.

• Extensive visual examinations of Penetration 42 (prior to and after residue sampling and cleaning) were performed. The examination indicated that the origin of the residue was not from the annular gap between the vessel head and the penetration tube. Evidence of previous above-head leakage, including pooling and dripping, was observed on the insulation panel seam in the vicinity above Penetration 42.

0 A volumetric examination, including ultrasonic/eddy current of the inside diameter of the penetration tube, was performed. No rejectable indications were found and no evidence of leakage in the shrink-fit region was observed in the J-groove weld inspection data.

& Supplemental eddy current examinations of the J-groove weld and penetration tube outside diameter surfaces were conducted. No rejectable indications were observed in the J-groove weld inspection data.

• A manual surface examination using liquid penetrant was performed on the Penetration 42 nozzle immediately above the reactor vessel closure head. No indications were observed.

A 100 % volumetric examination of the 78 control rod drive mechanism penetrations and the single vent penetration in the Unit 2 RPV head was completed in accordance with Section IV.C(5)(b)(iii) utilizing an approved relaxation of the requirements obtained in 8

accordance with Section JV.F. No unacceptable indications were found. No repairs of any head penetrations were needed or performed.

System pressure tests did not reveal any degradation of the system pressure boundary or the installed bolting.

COOK NUCLEAR PLANT, UNIT 2 BRIDGMAN, MI COMMERCIAL SERVICE DATE 7-1-1978 SECTION 6 ABSTRACT OF EXAMINATIONS, RESULTS, AND CORRECTIVE ACTIONS FOR ASME SECTION XI, SUBSECTIONS IWE AND IWL Containment Inservice Inspections During the reporting period of November 10, 2004, to May 4, 2006, inservice inspections were performed on the Unit 2 Containment pressure retaining structure in accordance with Subsections IWE and IWL of ASME Section XI, 1992 Edition and 1992 Addenda, and 10 CFR 50.55a.

The following attachment provides a listing of the examinations performed, results, and corrective actions, if any were performed. A General Visual Examination was performed on all accessible surfaces of the Containment liner along with examinations performed in conjunction with coating repairs, and other repair/replacement activities.

An acceptance criteria providing the acceptable minimum thickness of the containment liner was established for the examination of the concrete-backed liner. The examination results were compared to the acceptance criteria. All results were within the established acceptance criteria. No areas required to be classified as an Augmented Examination area in accordance with IWE-1240.

Containment concrete examinations are currently in progress as of this report date. No degradation has been found that impacts the structural integrity of the containment concrete. As of this report date, there have been no conditions noted in accessible areas that could indicate the presence or result in degradation to inaccessible areas.

Based upon the favorable results of this inspection and in accordance with the provisions of Technical Specification 4.4.5.3a, the Unit 2 steam generators continue to be on an inspection interval of up to 40 calendar months.
