Cook Nuclear Plant Unit 1, State of Michigan, National Board Number 20761 (Mich. M-09672-M), located in Bridgman, Michigan. The plant is owned by Indiana Michigan Power Company, P.O. Box 60, Fort Wayne, IN, 46801.
The unit's commercial service date is August 23, 1975, and the Authorized Code Inspector is Mr. M. K. Muterspaugh, who is employed by Factory Mutual Insurance Company of Johnston, Rhode Island.
The examinations, tests, replacements, and repairs performed, conditions observed, and corrective measures taken are summarized in the attached report.
ISI activities were performed in accordance with the rules and requirements of ASME Code Section XI, 1989, and 1992 Editions as applicable.

COMMERCIAL SERVICE DATE 8-23-1975 SECTION 1 INTRODUCTION This report summarizes the Inservice Inspection activities performed at the Donald C.
Cook Nuclear Plant, Unit 1, for the period of April 22, 2002 to June 09, 2002. This report addresses examinations and activities associated with the Third Ten Year Interval, First Inspection Period, Second Outage, Refueling Outage, Unit 1 Cycle 18. These activities were performed in accordance with the requirements of the ASME Boiler and Pressure Vessel Code,Section XI, Rules for Inservice Inspection of Nuclear Power Plant Components. The Third Ten Year Inspection Interval began July 1, 1996 and continues through February 28, 2010. The applicable code of record for ISI activities is ASME Code Section XI, 1989 Edition (No Addenda) and 1992 Edition, 1992 Addenda as applicable.
The purpose of this report is to satisfy the requirements of IWA-6000 for the submittal of an Inservice Inspection Summary Report at the completion of each refueling outage.
Examination and tests conducted since the preceding summary report are included in this report. Repairs and replacements (for Class 1 and 2 Components) are also included in this report per the requirements of the 1989 Edition (No Addenda) and the 1992 Edition, 1992 Addenda of ASME Code Section XI, IWA-6220.
Contents of this report have been reviewed and verified correct with supporting documentation available at the plant for review if required.

COMMERCIAL SERVICE DATE 8-23-1975 SECTION 3 ABSTRACT OF EXAMINATIONS, RESULTS, AND CORRECTIVE ACTIONS FOR CLASS 1 AND CLASS 2 COMPONENTS Inservice examinations were performed during the period April 22, 2002 to June 09, 2002. This report addresses examinations and activities associated with the Third 10-Year Interval, First Inspection Period, Second Outage, Unit 1 Cycle 18 Refueling Outage. All examinations were in accordance with the ASME Code Section XI 1989 Edition (No Addenda) and 1992 Edition, 1992 Addenda as applicable.
Examinations were scheduled and performed according to the Third Ten-Year Long Term Inservice Examination Plan for Class 1, 2, and 3 Systems and Components for Donald C. Cook Nuclear Plant, Unit 1. This is the currently approved ISI examination plan that identifies specific activities to be performed on a refueling outage basis.
The scope of examinations was intentionally minimal due to the need to concentrate resources on the examination of the Unit I Reactor Pressure Vessel for evidence of Primary Water Stress Corrosion Cracking (PWSCC). The examinations for PWSCC were performed and documented in response to NRC Bulletin 2001-01. All 78 Vessel Head Penetrations and the Head vent were examined using a combination of ultrasonic, surface, eddy current and visual techniques. No evidence of PWSCC was observed.
In addition to the reactor vessel examination for PWSCC, Reactor Coolant Pump #13 bolting was examined as well as RCPs #12, #13, #14 installed flywheels and the Spare flywheel for the Reactor Coolant Pumps. No indications were noted.
System pressure tests did not reveal any degradation of the system pressure boundary or the installed bolting.
There was no evidence of degradation observed in the Steam Generator Eddy Current examinations.
Nondestructive ISI examinations were performed by Scientech, Framatome and Indiana Michigan Power (IMP) personnel in accordance with procedures approved for use at Cook Nuclear Plant. Primary Water Stress Corrosion Cracking (PWSCC) examinations were performed by Westinghouse and Indiana Michigan Power (IMP) personnel in accordance with procedures approved for use at Cook Nuclear Plant.

COMMERCIAL SERVICE DATE 8-23-1975 SECTION 6 ABSTRACT OF EXAMINATIONS, RESULTS, AND CORRECTIVE ACTIONS FOR ASME SECTION XI, SUBSECTIONS IWE AND IWL During the reporting period of December 23, 2000 to June 09, 2002, inservice inspections were performed on the Unit 1 Containment pressure retaining structure in accordance with Subsections IWE and IWL of ASME Code Section XI, 1992 Edition, 1992 Addenda, and 10 CFR 50.55a.
The following attachment provides a listing of the IWE examinations performed, results, and corrective actions, if any were performed. An acceptance criteria providing the acceptable minimum thickness of the containment liner was established for the examination of the concrete-backed liner. The examination results were compared to the acceptance criteria. All results were within the established acceptance criteria and the areas did not require to be classified as an Augmented Examination area in accordance with IWE-1240.
A visual examination of the accessible concrete surfaces of the Unit I and Unit 2 Containment structures was performed in June through August of 2001 in accordance with Subsection IWL of ASME Code Section XI. Examination Report No. NED-2001 034-REP documents the conditions noted and the evaluation by the IWL Responsible Engineer. The conditions noted were acceptable with no immediate corrective actions required.

COMMERCIAL SERVICE DATE 8-23-1975 SECTION 7 ABSTRACT OF EXAMINATIONS, RESULTS AND ACTIONS FOR STEAM GENERATOR EDDY CURRENT EXAMINATIONS Eddy current inspections were conducted on each of the four steam generators during the U1 C 18 refueling outage. The base inspection consisted of a 50% (nominal) inspection using a bobbin coil probe. In addition to the base inspection, sample rotating coil inspections were conducted in two steam generators in areas of particular interest. The sample inspection included a 20% top of hot leg tube sheet region inspection in one steam generator and a 100% inspection of the inservice row I and row 2 U-bends areas in a second steam generator. In addition, select diagnostic rotating coil examination was performed of special interest bobbin coil signals i.e. manufacturing burnish marks, dents/dings etc. The following table provides a breakdown of the scope of the inspection.

37 13 13 23 Special Interest II No degraded or defective tubes were identified during any portion of the inspection and as a result no tubes were plugged. Individual steam generator results are discussed below.
SG 11 One tube (Row 6/Column53) was identified as having a minor wear (9% through wall) indication at the sixth hot leg support. This indication was identified during a previous (1997) inspection and shows essentially no change. Three tubes (R22/C50, R23/C50, and R42/C50) were reported as possible loose part indications at the top of the hot leg tube sheet during the sample rotating examination. Expanded inspections in the area of interest identified two additional tubes (R41/C51, R42/C51) as also having possible loose part indications. Subsequent efforts identified a small wire and sludge rock in these locations. The wire was later removed. No tube degradation was identified related to the loose part indications.
