Cook Nuclear Plant Unit 2 Form OAR-1 Owner's Activity Report This letter provides a copy of Form OAR-1 Owner's Activity Report for inservice inspection activities performed at Donald C. Cook Nuclear Plant (CNP) Unit 2 through the completion of the U2C21 refueling outage. This report meets the requirements of the American Society of Mechanical Engineers (ASME) Code Case N-532-4, "Repair/Replacement Activity' Documentation Requirements and Inservice Summary Report Preparation and Submission, Section Xl, Division 1."

This code case is used as an alternative to the requirements of the ASME Boiler and Pressure Vessel Code, Section Xl, Article IWA-6000 as approved by Regulatory Guide 1.147, "Inservice Inspection Code Case Acceptability, ASME Section XI, Division 1, Revision 16."

The Authorized Code Inspection Company is OneCIS Insurance Co., of Lynn, Massachusetts. The Authorized Nuclear Inspector is Reuel Schenck.
