Dear Mr. Denton:

This letter provides our final report (Attachment 1) of the Perry off-gas system charcoal adsorber combustion events that occurred on June 20 and July 6, 1986. Included in this report are the chemical and physical analysis results for the original charcoal as well as for the replacement charcoal; the metallurgical analysis results for the adsorber vessels; descriptions of the unloading, drying and reloading processes; the initiating events analyses; and other pertinent items. We have included several photographs of the vessels and the activities associated with recovery from these events. We have also included a separate attachment of the temperature and combustion products plots of both events (Attachment 2).

We are presently loading the replacement charcoal. The charcoal adsorber vessel trains will be helium leak tested following loading. The off-gas vault refrigeration system testing will then be completed. Additional evaluation is underway to verify the acceptability of the vessel weld metal. The evaluation will be completed this week and we expect it to show that the weld metal was unaffected by these events. If it proves otherwise, we will inform you.
