MISPOSITIONED INSTRUMENT VALVES On March 8,1986, during an operational readiness inspection, the licensee identified a drywell pressure instrument that was isolated by a mispositioned valve on a system that had been declared operable for fuel load. Because of this finding, the licensee expanded its review to determine if the incident was a single event or more widespread. The review uncovered 57 instances of instrument valves or valve caps in the wrong position. Almost all the valves (vent, drain, rack isolation) were in systems not yet declared operable.

Normally, these valves are not manipulated during an instrument calibration. The i mispositioning may have occurred during modification and maintenance activities following

, preoperational testing.

-l The licensee has initiated an extensive program to correct the problem, including j verification of position, tagging and revision of procedures and drawings. In addition, the

licensee will determine if past surveillance tests or preoperaticnal tests could have been affected by the mispositioned valves. To date, no tests were found to be affected.

The resident inspectors are following the licensee's course of action.

The State of Ohio will be notified.

Region III (Chicago) was informed of the multiple valve errors on March 12, 1986. This information is current as of 10 a.m. (CST), March 14, 1986.
