AUGMENTED INSPECTION TEAM TO REVIEW RECENT EQUIPMENT PROBLEMS R:gion III (Chicago) has dispatched an Augmented Inspection Team to review the circumstances surrounding a reactor trip which occurred on May 19, 1986. Although plant safety systems generally performed as designed during the reactor trip, several equipment problems occurred. These equipment problems, coupled with various problems that have occurred over the past 18 months, led to the formation of the Augmented Inspection Team.

The Augmented Inspection Team is composed of Region III personnel and representatives from the Office of Inspection and Enforcement, Office of Nuclear Reactor Regulation, and Office of Analysis and Evaluation of Operational Data. The team will focus on the plant's maint: nance activities and procedures for determining equipment operability.

A Confirmatory Action Letter was issued by Region III to the licensee on May 21, 1986, documenting the licensee's agreement to rtace the unit in cold shutdown, pending a review by th licensee of the May 19 event and an #',aluation of the operability of other safety-related and balance-of-plant equipnent. The licensee is also to notify Region III of c:rr:ctive actions taken as a result of these reviews and receive concurrence of the Regional Administrator for resuming operations.

In th? May 19 incident, the reactor tripped from high pressure after a loss of turbine control power. Equipment problems included the failure of the turbine bypass valve to open automatically, the failure of one atmospheric dump valve to open, failure of a letdown DISTRIBUTION:

Upon evaluation of the shutdown, the licensee declared an Unusual Event under its emergency

The licensee is planning to issue a news announcement on the shutdown and the pending NRC Augmented Inspection Tean review. A special review team has also been formed by the licensee.
