APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-289 Three Mile Island, Unit No.1 License No. DRP-50 As a result of an inspection conducted on March 7 to April 11, 1986; and, in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the follow-ing violations were identified:

A. 10 CFR 20.201(b) requires that each licensee make such surveys as may be necessary to comply with all sections of Part 20. As defined in 10 CFR 20.201(a), " survey" means an evaluation of the radiation hazards incident to the production, use, release, disposal, or presence of radioactive materials or other sources of radiation under a specific set of conditions.

Requirements of 10 CFR 20.103 (a)(3) in part are that "... the licensee

- shall use suitable measurements of concentrations of radioactive materials I in air for detecting and evaluating airborne radioactivity ... as may be necessary for timely detection and assessment of individual intakes of radioactivity by exposed individuals ...."

Contrary to the above, on March 24, 1986, the licensee failed to perform a timely evaluation of airborne radioiodine in the Reactor Building (RB).

Work on the once-through steam generators (OTSGs), initiated at approx-imately 9:00 a.m., generated airborne radioiodine in the RB which was not detected until approximately 11:00 p.m. The nearly fourteen (14) hour delay resulted in the unplanned exposure of seventy-seven (77) personnel, with intakes of radioactive iodine as high as 120 nCi.

This is a Severity Level IV violation (Supplement IV).

B. 10 CFR 20.203(b) requires that each radiation area shall be conspicuously posted with a sign or signs bearing the radiation caution symbol and the words, " Caution - Radiation Area."

10 CFR 20.202(b)(2) defines a Radiation Area as "any area accessible to personnel in which there exists radiation ... at such levels that a major portion of the body could receive in any one hour a dose in excess of 5 millirem ..."

Contrary to the above, on April 1,1986, at approximately 3:00 p.m. , the radiation area in the hot machine shop on the 305-foot elevation of the Auxiliary Building was not conspicuously posted as required. Survey measurements inside the hot machine shop indicated general area dose rates up to 20 millirem / hour.

O Appendix A 2 Pursuant to the provision of 10 CFR 2.201, GPU Nuclear Corporation is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compli-ance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
