Inspection 50-289/86-05
During the period from March 7, 1986, to April 11, 1986, members of the NRC
Region I Staff conducted routine safety inspection activities at your facility
with respect to power and shutdown operations, radiation protection and fire
protection. The inspectors documented the results of these inspections in the
-
enclosed inspection report. At the conclusion of the inspection, Mr. R. Conte,
of my staff, summarized the inspection findings for you and other members of
your staff. We recognize several positive attributes and initiatives on your
part in controlling your transition to, and the performance of, outage work.
However, there were two apparent violations of NRC requirements (Appendix A)
identified during the outage period.
In one instance there was a failure to properly survey for airborne activity,
and this resulted in Reactor Building (RB) iodine concentrations going unde-
tected for up to fourteen hours, along with unplanned exposures to personnel,
when the reactor coolant system was vented to the RB.
In another instance,
there was a failure to properly post a radiation area. You are required to
respond to this letter; and, in so doing, you should follow the instructions
in Appendix A.
With respect to the radiological events described in paragraphs 3.3 and 3.4 of
the inspection report, we note that enhanced personnel and management attention
to circumstances leading to these events may have precluded them from occurring.
In responding to the violations, you should also provide your view on the pace
of activities as a contributing factor to these events and, as applicable, pro-
vide corrective measures to prevent recurrence.

This letter also acknowledges receipt of your letter, dated January 31, 1986,
concerning the event in which records were discarded for the Power Operated
Relief Valve (PORV) surveillance test data.
Your response addressed the con-
cerns we had about that incident. We will review your corrective action in a
future inspection.
