NOTICE OF VIOLATION D

f GPU Nuclear Corporation Docket No. 50-320 Three Mile Island Unit.2 License.No. DPR-73 As a; result of the inspection conducted August 1, through August 28, 1987, and in accordance with the " General Statement of Policy and Procedure for NRC

. Enforcement Actions",10 CFR Part 2, Appendix C (Enforcement Policy 1986), the

, following violations were identified:

L A.- Technical Specification 6.8.la requires, in part, that " Written procedures be implemented for tasks recommended by' Regulatory Guide 1.33, Quality Assurance' Program Requirements (Operation), Revision 2, February 1978, Appendix'"A". Within' Appendix A, Section 7.b(1) lists spent resins and filter sludge handling as a subset of the solid waste system in Procedures for Control ~of Radioactivity (for limiting materials released to the environment and limiting personnel exposure).

Contrary to the above, on August. 13, 1987, personnel did not adequately implement the requirements of Procedure 4231-0PS-3233.02, Operation of the Chem-Nuclear CementFSolidification Unit 125, Revision 0-00, dated January 20, 1987.- Sections 4.0, 4.1, and 4.3.7 in Attachment 1~of this procedure, in sum, require the operators to. verify the proper connection of all temporary hoses used with the Unit to accept sludge from.the plant and pump'the sludge to the solidification liner. Lacking this verification, processed water passed through the system for hydrostatic test purposes carried residual auxiliary building sump sludge through an unconnected hose to the auxiliary building floor, contaminating the area with radioactive material. The contamination produced radiation levels of up' to 20 rad /per hour, and required in excess.of one week to decontaminate.

This is a Severity level IV violation (Supplement IV).

B. 10 CFR 20.201(b) requires that each licensee make or cause to be made such surveys as (1) may be necessary for the licensee to comply with the regulations in 10 CFR Part 20, and (2) are reasonable under the circumstances to evaluate the extent of the radiation hazards that may be present.

Contrary to the above, on August 19, 1987, the licensee did not adequately evaluate the radiation hazards incident to the presence of radioactive material in waste packages containing contaminated sample tubing resulting in the contamination of an individual, other packages,

-and the transporting vehicle with the potential for the spread of contamination to unrestricted areas.

This is a Severity level IV violation (Supplement IV).

Pursuant to the provision of 10 CFR 2.201, the GPU Nuclear Corporation is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, includirg: (1) the corrective steps which have been taken and the results achievect (2) corrective steps which will be taken to avoid further

. violations; and (3) the date when full compliance will be achieved. Where

. good cause is shown, consideration will be given to extending this response time.
