APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-289 Three Mile Island, Unit 1 License No. OPR-50 During an NRC inspection conducted between June 12 - July 15,1988, one violation of NRC requirements was identified. In accordance with the "General Statement of  ;

Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is set forth below.

The 10 CFR Part 50, Appendix B, Criteria V1, "Document Control," and the lic-ensee's NRC-approved Operational Quality Assurance Plan, Revision 1-00, Sec-tion 3.2.1, and ANSI-18.7-1976, Section 5.2.15 require that measures shall assure that documents, including changes, are reviewed for adequacy and ap-proved for release...and are distributed to...the location where the pre-2 scribed activity is performed. The Administrative Procedure No. 1065, "Vendor Document Control," Revision 2, Section 5.0, requires use of controlled manuals t for construction activities.

Contrary to the above, between June 23 and 24,1988, the below-noted plant construction activity was conducted without the use of the controlled copy of the vendor manual (VM-TM-1024) in the work package. The Job Order No.

A25A-V1214 was the "Replacement of Fan Blades on the AH-E-95 A & B Fans,"

(part of the control building ventilation system). As a result, the fan blades were improperly torqued and the incorrect fan hub was described in the installatic' instructions.

I Pursuant to the provision of 10 CFR 2.201, GPU Nuclear Corporation is hereby re-J quired to submit to this office, within thirty days of the date of the letter transmitting this notice, a written statement or explanation in reply, including

for each violation

(1) the corrective steps which have been taken and the results 1

achieved; (2) the corrective steps which will be taken to avoid further violations; and, (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time. ,
