l APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-289 Three Mile Island, Unit No. 1 License No. DRP-50 As a result of an inspection conducted on October 2-31, 1987, and, in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

Technical Specification 6.8.1 states, "Written procedures important to safety shall be implemented... covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978...." Appendix A of Regulatory Guide 1.33, Revision 2, lists procedures for bypass of safety functions and jumper control" and also for "surveillance tests and calibrations." Step 6.14 of Preventive Maintenance Procedure IC-15, Revision 5, dated February 20, 1986, associated with the calibration check of nuclear river water pump differential pressure (D/P) switches requires the use of a jumper and lifted lead verification per AP 1013, Enclosure 5 for important-to-safety equipment.

Contrary to the above, on October 14, 1987, leads for important to safety D/P switch NR-DPS-138 were lif ted and reconnected without completion of the requirements of AP 1013, Enclosure 5. Additionally, the appropriate step of procedure IC-15, Step 6.14, was marked "N/A" when in effect it should have been performed as required by 10-15.

This is a Severity Level V violation (Supplement I).

Licensee corrective actions taken or planned are adequate as documented in the attached inspection report. Accordingly, no response to the violation is required.
