APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-289 Three Mile Island, Unit 1 License No. OPR-50 During an NRC inspection conducted between July 16 and August 27, 1988, two viola-tions of NRC requirements were identified. In accordance with the "General State-ment of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violations are set forth below.

A. TMI-1 Technical Specifications (TS), Section 3.1.12.3, states that, "If the reactor vessel head is installed and Taye is less than 275 F, high pressure injection pump breakers shall not be racked in unless: (b) pressurizer level is less than 220 inches." FurtTer, TS Section 3.1.2.3, limits the cool down rate of the pressurizer to 100 F in any one hour.

Contrary to the above, on June 19, 1988, at about 5:30 a.m., with Tave at 150 F, the pressurizer level increased to 275 inches with the reactor vessel head installed and high pretsure injection pump breakers racked in (one pump oper-ating); and, between 4:30 a.m. and 5:30 a.m., the pressurizer temperature was allowed to decrease by 160 F.

This is a Severity Level IV violation (Supplerrent I).

No response to Violation A is required.

B, Technical Specification, Section 6.8.2, requires that once the written proce-dures are estabitsbed, any substantive changes thereto shall be reviewed and approved ptfor to implementation. Corrective Maintenance Procedure 1407-1, Revision 32, paragraph 6.2.1.2, states, "where changes to approved procedures are required, these changes may be made by addendum to the maintenance proce-dure. The same level of approvals are required for the addendum."

Contrary to the above, on July 29, 1988, the scop 6 of the maintenance proce-dure, Job Ticket No. CS-956 involving torquing of the pressurizer code safety relief valve, RC-RV-1A tailpipe support (PR-32) bolts was substantively changed and implemented without obtaining the same level of review and ap-proval as the original job ticket.

This is a Severity Level V violation (Supplement I).

Pursuant to the provision of 10 CFR 2.201, Gpu Nuclear Corporation is hereby re-quired to submit to this office, within thirty days of the date of the letter transmitting this notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and, (3) the date when full compliance will be achieved. Where good cause is shoat, con',ideration will be given to extending this response time. This response is only required for violation B.
