APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-320 Three Mile Island Unit 2 License No. DPR-73 As a result of the inspection conducted February 14 through March 31, 1987, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C (Enforcement Policy 1986), the following violations were identified:

A. Technical Specification 6.8.1 states " Written procedures shall be... implemented... covering the... surveillance and test activities of safety-related equipment..." Surveillance procedure 4210-SUR-3811.04,

" Fire System Deluge / Sprinkler System Functional Test" requires that operating procedure 4210-0PS-3810.01 " Fire Protection System" be implemented to return-to-service the deluge system for the Control Room ventilation system.

Contrary to the above, on January 23, 1987, operating procedure 4210-0PS-3810.01 was not implemented when returning the deluge system for the Control Room ventilation system to service which resulted in the Control Room Emergency Air Cleanup System being inoperable from 0850 hours0.00984 days <br />0.236 hours <br />0.00141 weeks <br />3.23425e-4 months <br /> on January 23, 1987 to 0120 hours0.00139 days <br />0.0333 hours <br />1.984127e-4 weeks <br />4.566e-5 months <br /> on January 26, 1987.

This is a Severity Level IV violation (Supplement I).

B. 10 CFR 20.207 requires that licensed materials in an unrestricted area and not in storage be tended under the constant surveillance and immediate control of the licensee.

Contrary to the above, the licensee did not tend and keep licensed radioactive materials under constant surveillance and control in that on February 25, 1987, radioactive material was left untended in unrestricted areas both on the licensee's plant site and in public, offsite, areas and on March 18, 1987, radioactive material was left untended in a vehicle in an unrestricted area on the licensee's plant site.

This is a Severity Level IV violation (Supplement IV).

Pursuant to the provision of 10 CFR 2.201, the GPU Nuclear Corporation is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
