50-289 Three Mile Island Nuclear Station License No. DPR-50 i
j As a result of the inspection conducted from August 7 to September 21, 1990, e
and in accordance with the " General Statement of Policy and Procedure for NRC i
Enforcement Actions," 10 CFR Part 2, Appendix C, the following violation was identified:
Technical Specification 6.8.1 requires that written procedures shall be established and implemented for surveillance and test activities of equipment that affects nuclear safety and for maintenance that can affect the l
performance of safety-related equipment.
l Contrary to the above:
a.
On August 22, 1990, the licensee failed to adequately establi:;h Surveillance Procedure (SP) 1303.4.1 " Reactor Protection System."
Step 8.6 did not provide adequately establish the initial conditions for testing RPS response. The step failed to reset the bistable after testing the previous function.
This could have resulted in a Reactor Protection System function not being fully tested, b.
On September 10, 1990, the licensee failed to adequately implement Preventative Maintenance Procedure E-18 " Battery Chargers Annual Inspection." The prerequisite to perform the specific checklist required by step 1.1.1 was not accomplished.
This is a Severity Level IV violation (Supplement 1)..

(1) admission or denial of the violation; (2) the reasons for the violation, if admitted; (3) the corrective steps which have been. taken and the results achieved; (4) corrective steps which will be taken;to avoid further violations; and (5) the date when full compliance will be achieved.
