MONTHLY NARRATIVE REPORT OF OPERATION AND MAJOR MAINTENANCE EXPERIENCE This report describes the operating and major maintenance experience for the month of September, 1986. This narrative report was prepared by the plant staff and is submitted in accordance with Section 6.9.1.7 of the Units 1 and 2 Technical Specifications.

On September 1, 1986 Unit I started the month shutdown for its first refueling outage. Unit 2 started the month at 100% power.

On September 6, 1986 Unit 2 reduced power to 70% for Main Steam Isolation Valve part-stroke testing and returned to 100% power.

On September 8, 1986 Unit 1 experienced a loss of decay heat removal capability when a voltage transient in the solid state protection system caused one of the RHR suction valves to close.

On September 14, 1986 Unit 2 reduced power to 50% to plug leaking main condenser tubes.

On September 15, 1986 While preparing to inc'rease power to 100% after plugging leaking main condenser tubes, the motor thrust bearing on Circulating Water Pump 2-1 failed. Unit 2 was restricted to 50% power until the pump was returned to service.

On September 16, 1986 Unit 1 experienced an ESF actuation when a spurious high level alarm on RM-25 caused the control room ventilation system to shift to Mode 4 on both units.

On September 17, 1986 Unit 2 returned to full power.

On September 18, 1986 Unit 2 experienced a reactor trip and safety injection when adjustment of the main feedwater pump 2-2 controller caused the pump to runback to minimum speed.

On September 18, 1986 Unit 2 experienced a reactor trip from Mode 3 (Hot Standby) while checking lug screw tightness in the reactor trip breaker cubicles.

On September 20, 1986 Unit 2 was synchronized to the PGandE system.

On September 21, 1986 Unit 2 returned to full power.

On September 23, 1986 Unit 2 reduced power to 50% to investigate main condenser tube leakage.

On September 24, 1986 Unit 2 returned to full power.

On September 24, 1986 Unit 2 returned to full power.

On September 26, 1986 Unit 2 reduced power to 50% to investigate main condenser tube leakage.

On September 27, 1986 Unit 2 returned to full power.

On September 30, 1986 Unit 1 ended the month shutdown for refueling.

Unit 2 reduced power to 50% to investigate main condenser tube leakage and ended the month at 50%

power.

Unit I was continuing its first refueling outage which started August 29.

The first fuel assembly was removed from the reactor on September 30.

Unit 2 operated this month with a unit availability factor of 91.7% and a unit capacity factor of 78.8%. Unit 2 reduced power five times this month, once for main steam isolation valve testing and four times to investigate and repair main condenser tube leaks. In addition, Unit 2 experienced a reactor trip and subsequent safety injection from power and a reactJr trip from Mode 3.

No ciiallenges to the steam generator safety valves or pressurizer power operated relief valves have been made. Unit I has numerous safety related maintenance. activities performed in support of the refueling outage, including:

o Steam generator feedwater sparger "J" tube replacement o Steam generator eddy current tube inspection o Steam generator sludge removal o Reactor disassembly for refueling o Overhaul of two containment fan cooler unit motors o Nuclear instrument inverter changeout of inverters 1-3 and 1-4 o Installation of redundant overcurrent devices for containment penetration conductors No major safety related maintenance was performed on Unit 2 this month.
