MONTHLY NARRATIVE REPORT OF OPERATION AND MAJOR MAINTENANCE EXPERIENCE This report describes the operating and major maintenance experience for the month of August,1986. This narrative report was prepared by the plant staff and is submitted in accordance with Section 6.9.1.7 of the Units 1 and 2 Technical Specifications.

On August 1, 1986 Unit I started the month at 61% power while coasting down for its first refueling outage. Unit 2 started the month at 75% power while investigating a saltwater leak in the main condenser.

On August 2, 1986 Unit 2 returned to 100% power.

On August 3/4,1986 Unit 2 reduced power to 50% power while plugging leaking condenser tubes and returned to 100% power.

On August 9, 1986 Unit 2 reduced power to 35% power for scheduled demusseling.

On August 11, 1986 Unit 2 returned to 100% power.

On August 29, 1986 Unit 1 experienced a reactor trip due to a loose connection that was disturbed during a shunt trip functional test of a reactor trip breaker. Since the trip occurred within eight hours of the scheduled shutdown for the first refueling outage, there was no attempt to restart the reactor.

On August 31, 1986 Unit 1 ended the month shutdown for refueling. Unit 2 ended the month at 100% power.

Unit 1 operated this month with a unit availability factor of 92.1% and a unit capacity factor of 48.6%. On August 29, Unit 1 experienced a reactor trip and commenced its first refueling outage after a record setting run of 165 days continuously synchronized to the PGandE system.

Unit 2 operated this month with a unit availability factor of 100% and a unit capacity factor of 94.9%. Unit 2 reduced power twice this month, once to plug leaking main condenser tubes and once for demusseling.

No challenges to the steam generator safety valves or pressurizer power operated relief valves have been made. No major safety related maintenance was performed on either unit this month.

Prevent Recurrence 1 8/29/86 F 59.1* H 3 1-86-010 JC CON The reactor tripped while technicians were connecting meter test leads for testing the shunt trip function on a reactor trip breaker. This event was caused by loose connections on a relay in the reactor trip breaker cubicle, that were disturbed by the technician. The loose connections found as a result of this event were tightened. To prevent recurrence, similar connections on the other reactor trip breakers are being inspected for tightness and a sample of connections on other similar relays is being inspected to assure that the problem is not generic.

O Since this reactor trip occurred within eight hours of the scheduled shutdown for this unit's first refueling outage, there was no attempt to restart the reactor.
