APPENDIX A NOTICE OF VIOLATION Wolf Creek Nuclear Operating Corporation Do b t: 50-482 Wolf Creek Generating Station License. NPF-42 During an NRC inspection conducted during May 15 through June 30, 1988, one violation of NRC requirements was identified. The violation involved the failure to follow procedure. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988),theviolationislistedbelow.

A. Failure To Fellow Procedure Technical Specification 6.8.1 requires that written procedures be established, implemented, and maintained for the fire protection program.

Proceiure ADM 13-103, Revision 5, "Fire Protection: Impairment Control,"

implements procedures for impaired fire protection equipment including degraded fire doors. ADM 13-103, Section 4, requires that fire protection impairment control permit forms be preoared for all fire protection system l impairments. In addition, Section 4 requires that entries be made in the fire protection impairment control log for the impairments.  ;

1 Contrary to the above, on June 6,1988, Fire Doors 13261 and 13281 were observed to be standing open. Neither fire protection impairment control permits nor log entries had been completed for those doors.

This is a Severity Lesel IV violation. (Supplement I) (482/8819-01)

Pursuant to the provisions of 10 CFR 2.201, Wolf Creek Nuclear Operating )

Corporation is hereby required to submit to this Office, within 30 days of the  !

date of the letter transmitting this Notice, a written statement or explanation l in reply, including for each violation: (1) the reason for the violations if l admitted, (2) the corrective steps which have been taken and the results I achieved, (3) the corrective steps which will be taken to avoid further I violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
