NOTICE OF VIOLATION M'ri Steve Buehne ,

IA 98-034 During an NRC investigation completed February 6,1998, a violation of NRC requirements was

identified.- ln accordance with the " General Statement of Policy and Procedure for NRC 1l Enforcement Actions," NUREG-1600, the violation is listed below:

10 CFR 50.5 states, in part, that any licensee or any employee of a licensee may not deliberately submit to the licensee information that the person submitting the information knows to be incomplete or inaccurate in some respect material to the NRC.

l Contrary to the above, on December 11,1996, Steve Buehne intentionally submitted information to the licensee that he knew was inaccurate, and such information was material to the NRC. Specifically, Mr. Buehne deliberately falsified the signature of a welding engineer on a field welding material requisition (FWMR) form. This information is material because it is required by the licensee's welding program and by technical specifications in order to ensure that welding materialis of sufficient quality when used I on equipment that is important to safety or safety-related. (01014) .

This is a Severity Level IV violation'(Supplement 1).
