, 1 APPENDIX A NOTICE OF VIOLATION Wolf Creek Nuclear Operating Corporation Docket: 50-482 Wolf Creek Generating Station License: NPF-42  ;

During an NRC inspection conducted August 1 through September 11, 1993, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

Inadeauate System Operating Procedure Technical Specification 6.8.1.a states that written procedures shall be ,

established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, dated February 1978. Regulatory Guide 1.33, Appendix A, Item 3.d, requires procedures for filing, venting, and startup of emergency. core cooling systems.

10 CFR Part 50, Appendix B, Criterion V, " Instructions, Procedures,'and '

Drawings," requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type approrriate to the ,

circumstances.

i Procedure SYS EP-200, " Accumulator Safety Injection Operation," Revision 11, Section 4.1.1, specified instructions for increasing safety injection accumulator tank level in Modes 1, 2, or 3 (above 1700 psig) using either safety injection pump.

Contrary to the above, Procedure SYS EP-200 did not require initiating cooling water flow to the safety injection pump bearing oil heat exchanger and, on August 13, 1993, operators started a safety injection pump without starting a component cooling water pump.

This is a Severity Level IV violation. (Supplement I) (482/9324-01) ,

A written response to the violation discussed above is not required. This was based on the actions taken by the licensee to determine the root cause and -

implement the appropriate corrective actions.
