ENCLOSURE 1 NOTICE OF VIOLATION .

Wolf Creek Nuclear Operating Corporation Docket No.: 50-482 Wolf Creek Generating Station License No.: NPF-42 EA No.: 98-099 During an NRC inspection conducted June 24-27,1997, at your Wolf Creek Generating Station reactor facility, with in-office inspection continuing until July 8,1997, unresolved items were identified. Subsequent NRC review of these items has identified two violations of NRC requirements. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violations are listed below:

Technical Specification 6.8 requires establishment, implementation, and maintenance of procedures covering the activities referenced in Appendix A of Regulatory Guide 1.33, Revision 2 dated February 1978.

Section 9 in Appendix A of Regulatory Guide 1.33, addresses procedures for maintenance activities, including control of maintenance, repair, replacement, and modification work.

A. Section 5.7 in Procedure AP 16C-002, " Work Controls," Revision 5, states that supervisors and workers are responsible for ensuring that work is performed in accordance with the requirements of the work package task and applicable procedures.

Section 5.9 states that supervisors and workers are responsible for ensuring that work performed is documented in the completion / history sections of the task package.

Contrary to the above, the licensee identified that unauthori?.ed work (i.e., work outside the scope of assigned work package tasks and applicable procedures) was performed and such work was not documented when on:

1. March 28,1996, alteration (grinding) of vendor-installed welds on a safety-related essential service water system elbow on the Train A component cooling water room cooler occurred, and
2. Approximately August 29,1996, base metal weld repair (plug welding) performed on a safety-related normal charging pump baseplate occurred.

This is a Severity Level IV violation (Supplement I) (50482/9713-05).

B. Ssetion 5.4.11 in Revision 6 to Procedure FMC-1, " Filler Material Control," states that filler material stubs and damaged filler material shall be placed in stub buckets by the welder, and.the stub buckets shall be retumed daily to the point of issue where the filler material was issued. Section 5.5 states that welders shall retum unused weld filler material to the point ofissue.

I j Contrary to the above, the licensee identified during February and March 1997, that its l

i welders had not retumed unused weld filler materials to the point of issue, when it discovered 5-stainless steel welding rods and 10-E7018 covered electrodes in the weld l shop area.

l l This is a Severity Level IV violation (Supplement 1) (50-482/9713-06).

1 Pursuant to the provisions of 10 CFR 2.201, Wolf Creek Nuclear Operating Corporation is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555, with a copy to the Regional Administrator, Region IV,611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response. If an adequate reply is not received within the time specified in this Notice, an order or a Demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be l proper should not be taken. Where good cause is shown, consideration will be given to l extending the response time.
