APPENDIX A NOTICE OF VIOLATION l

Wolf Creek Nuclear Operating Corporation Docket: 50-482 l Wolf Creek Generating Station License: NPF-42 l

During an NRC inspection conducted April 1-30, 1989, one violation of NRC requirements was identified. The violation involved inadequate corrective action. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

Inadequate Corrective Action 10 CFR Part 50, Appendix B, Criterion XVI, " Corrective Action," requires, in part, that measures shall be established to assure that, for significant conditions adverse to quality, the cause of the condition is determined and corrective action taken to preclude repetition.

NRC Enforcement Action EA 87-213, issued March 17, 1988, Violation A.

" Failure to Follow Procedure," Example 6.c cited the licensee for a welder performing a weld which he was not qualified to perform. The licensee's response dated April 14, 1988, stated that corrective steps, which included qualification review by supervisors prior to assignment of welders, would be achieved by June 15, 1988.

Contrary to the above, the corrective actions were not effective in precluding repetition in that on March 24, 1989, the licensee identified  !

that a nonqualified welder had performed welding on a portion of safety-related piping, in November 1988, specifically the essential service water (ESW) return to the ultimate heat sink.

This is a Severity Level IV violation. (Supplement I)(482/8913-01)

Pursuant to the provisions of 10 CFR 2.201, Wolf Creek Nuclear Operating l j

Corporation is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
