APPENDIX A-NOTICE OF VIOLATION  ;

Wolf Creek Nuclear Operating rorporation Docket: 482  !

Wolf Creek Generating Station License: NPF l During an NRC inspection conducted August I through September 14, 1990, two.

violations of HRC requirements were identified. The violations involved failure to have an adequate procedure and failure to take adequate corrective  ;

action. In accordance with the " General Statement of Policy and Procedure for '

NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1990) (Enforcement Policy),

de violations are listed below:

A. D ilure to Have an Approved Procedure Technical Specification 6.8.1 requires approved written procedures for activities affecting safety. WCGS administrative procedure ADM 07-100, Revisio. 45, " Approval and Distribution of WCGS Proceduros," paragraph 4.3.1, requires, in part, that Revision 0 of nonnuclear safety-related procedures-be approved by the plant safety review committee (PSRC) to ensure the procedure is appropriately classified and that it receive plant manager approval sign (ture.

This is a Severity Level IV violation. (Supplement I) (482/9031-01)-

B. Failure to Take Adequate Corrective Action Criterion XVI of 10 CFR Part 50, Appendix B,'" Corrective Actions,"

requires, in part, that measures shall assure.that conditions adverse to quality are promptly identified and correctea. The licensee's quality assurance Procedure QAP 16.1, " Corrective A: tion:fcr QA Program  ;

Breakdowns," Revision 3, paragraph 6, statts that corrective action . I controls, as established in fthis procedure, 'shall assure that -significant' ,

conditions adverse to quality are promptly identified, reported, and corrected to preclude recurrence as required by WCNOC Corporate Quality Manual, KGP-1210 and ADM 01-033. Procedare KGP-1210, Revision'3,

" Programmatic Deficiency Reporting." Section 6.1 requires a programmatic deficiency repcrt be written for a significant deviation from expected plant performance.

Pursuant to the provisions of 10 CFR 2.201, Wolf Creek Nuclear _0perating Corporation is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation, in reply, including for each violation: (1) the_ reason'for the violation if admitted, (2) the corrective steps which have been taken and the results ,

achieved, (3) the corrective steps which will be taken to-avoid further '

violations, and (4) the date when full compliance will be' achieved. Where good cause is shown, consideration will be given to extending the response time. .
