4 NJPDES 8/22/91 Explanation of conditions July 1991 The following explanations are includod to clarify possiblo deviation f rom permit conditions.

General - The coluinns labeled "No.

Ex", on the onclosed DMR, tabulate the number of daily dischargo values outsido the indicated limits.

Data reporting and accuracy reflect the working environment, the design capabilities and rollability of the monitoring instruments and operating equipment.

Deviations from required sampling, analysis monitoring and reportino methods and periodicition are noted on the respectivo DMR.

Frequency for dischargo point 463A, the cooling Tower Blowdown, and the River were duno at approximately S hour intervals to provide for the cycles of concentration in the system.

As por the Administrativo Consent Order the TSS limit for dischargo points 462A, 463A and 464 have boon lifted and the interim thermal limits for dischargo point 461A have boon changed to 443 MBTU/hr (Jeno - September) and 731 MBTU/hr (October - May).

EXPLAllATION 461A On the 26th of July a viuible sheen was noted on the Delaware River emanating from the CTB dischargo line. This was attributed to a release of less than 1 gallon from the LVOW system.

Box 2809 Trenton, New Jersey 08625 Gentlemen HOPE CREEK GE!!ERATING STATION NJPDES NJ0025411 OIL RELEASE On July 26, 1991, a suspected release to the Delaware River of less than 1 gallon of fuel oil was reported at the Hope Creek Generating Station.

This was reported to the NJDtP Hotline (Case, 91-07-26-1509-30) and the National Response Center (Case 81150).

Release n:timation was based on sheen iridescence and size (30 feet wide X 180 feet long).

This report has been prepared to satisfy the requirements under NJAC 7:1E-2.2 and NJP0ES Permit NJ0025411, Part I, 14.D.

BACKGROUND This facility user various oils throughout the site.

Oily wastes are collected and directed to a Low Volume Oily Water (LVOWS) treatment system, DSN 461C, prior to discharge to the Delaware River via the Cooling Tower Blowdown discharge line, D8N 461A, in accordance with HJPDES Permit NJ0025411.

Petroleum Hydrocarbons is a permit limited constituent of this outfall.

The LVOWs is an API-type separator designed for removal of solid and floata M.natorials utilizing gravity separation.

Influent flown are empendent upor, water collection rates.

y EVENT DESuurr10N At approximately 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br /> on July 26, 1991, personnel observed a sheen on the Delaware River in the vicinity of Hope Creek Generating Station and reported to the Control Room operators.

Thay in turn notified the site Fire Department at 1335 hours0.0155 days <br />0.371 hours <br />0.00221 weeks <br />5.079675e-4 months <br />.

The Fire Department responded to the scene and by 1352 hours0.0156 days <br />0.376 hours <br />0.00224 weeks <br />5.14436e-4 months <br /> confirmed the presence of an oil sheen in the-process of breaking gy&%

They notified the Licensing and Regulatior2 Department l

requesting regulatory follow-up assistance.

Rolaase source identification was unconfirmed until 1502 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.71511e-4 months <br /> when it was determined that the probable source was the LVoWS.

Even though the actual release had self terminated by that time, the system and its feeder lift stations were shutdown pending further investigation.

Residual fuel oil, found in an upstream sump, may have precipitated the event.

Less than 1 gallon of oil was estimated released through the NJPDES permitted diccharge 461A during the course of this event.

In consideration of the short term of the event, the small quantity involved ano the particular discharge path, identification and corrective actions were accomplished in a timely manner.

SUMMARY

In summary, this incident involved the release of less than 1 gallon of fuel oil from the LV0WS, DSN 461C, which discharges to cooling Tower Blowdown, DSN 461A and subsequently to the Delaware River.

l A sheen, initially observed at approximately 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br /> on July 26, 1991, was determined at 1502 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.71511e-4 months <br /> to be a discharge which had terminated and was reported as such to the NJDEP Hotline and National Response Center at approximately 1510 hours0.0175 days <br />0.419 hours <br />0.0025 weeks <br />5.74555e-4 months <br />.

The event lasted approximately one hour and terminated prior to discovery.

Plart personnel reviewed plant operating records without identifying a clear cause.

At this time, it appears to be related to free fuel oil entering the system temporarily overloading the LVOWS and causing a bypass.

The contents of the separator and all related equipment has been removed for disposal at a licensed off-site hazardous waste management"feellity and'the separator disassembled and cleaned I

prior to besay returned to service.

After reassembly, the treatment app 4em was returned to service oa July 30, 1991 and has operated in compliance with its discharge limitations.
