OPERATOR AND REACTOR OPERATOR INITIAL EXAMINATION REPORT

50-354/98-302 - REPORT SUPPLEMENT

Dear Mr. Keiser:

This report supplements the first report issued February 26,1999, and provides the results of

individual operator examination appeals and the results of additional staff review on two facility

r:roblems: (1) the quality of initial examination submittals; and (2), a grouping of operator test

results which were close to the pass / fail demarcation of 80 percent. This review addressed

areas important to public health and safety in that the NRC staff reviewed the adequacy of the

initial license training program at your facility with respect to the two problems. These problems

were noted in both the February 1998 and December 1998 examinations.

Because of the repetitiveness of the two problems and other issues, your organization and the

NRC staff decided to conduct independent self-assessments for underlying causes and to share

results in a public meeting. In our meeting of March 22,1999, we discussed the results of these

self-assessments.

Subsequent to the meeting, we reviewed your root cause analysis for the problems, and we

discussed the results of that review with your representative in a telephone call of July 20,1999,

when individual operator appeal results were announced. We noted that your 1999 root cause

,

analysis for the two problems did not specifically identify why the problems from the February

1998 exam manifested themselves again on the December 1998 exam. We concluded in the

enclosed report that the implementation of your condition reporting process for the above two

problems was deficient, in that corrective' actions for those problems were not effective. We

plan additional review in this area.

- Wdh respect to individual operator appeal results, one reactor operator (RO) and one senior

.

reactor operator (SRO) of the four appellants received passing scores on the written test, and

they received NRC licenses. Overall,11 of 16 applicants received NRC licenses.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosures will be piaced in the NRC Public Document Room.
