Several parameters on each battery cell were measured and recorded on a data sheet and attached to the PM work order. The PM was completed and aigned-off as approved by the electrician's foreman.

On 12-06-89, the system engineer was reviewing the work order and found that the measured voltage of one cell was recorded as 2.10 volts. Technical Specifications (TS) require entry into a Liraiting Condition for Operation (LCO) when the voltage is not greater than 2.10 volts. Therefore, a TS violation occurred because-a LC0 action statement was not completed within the allowable time.

The causes of this event were a cognitive personnel error and an inadequate procedure. Although the voltage limit identified in the procedure was not met, the maintenance foreman erroneously approved the measurements and failed to alert the Shift Supervisor of the abnormal parameter. The foreman has been counseled and a memo will be sent to other appropriate personnel describing this event and the need for adequate reviews of documentation. Secondly, the procedure used did not indicate that cell voltage limits were tied to TS requirements. This procedure will be changed.

A. ~ REQUIREMENT FOR REPORT I This report is required per 10.CFR 50.73 (a)(2)(i) because the unit operated ,

in a condition outside of Technical Specification (TS) requirements. A Limiting Condition for Operation (LCO) action statement was not completed within the allowable time when it was found that a component was not meeting its operability requirements.

-B. UNIT STATUS AT TIME OF EVENT At the' time of the occurrence of this event on 11-22-89, Unit I was o)erating in Mode 1/(Power Operation) at 100% of rated thermal power. Other ,

tlan that described herein, there was no inoperable equipment which contributed to the occurrence of this event.

C. DESCRIPTION OF EVENT 1 On-ll-22-89, _ an electrician was performing monthly preventive maintenance (PM)- on battery bank 1001B in the 125 volt DC Class lE electrical system per Procedure 27915-C, " General Battery Maintenance". Several parameters on each battery cell were measured and recorded on the " Battery Maintenance Data Sheet" attached to the PM work order. The PM was completed and signed-off as approved by the electrician's foreman.

On 12-06-89, at approximately 1330 CST, the system engineer was reviewing the work order and found that the measured voltage of cell #9 was 2.10 volts. Technical Specification Table 4.8-2 requires the float voltage for each connected cell to be greater than 2.10 volts. If this condition is not '

met, the applicable action statement requires that the battery bank be restored "to OPERABLE status within 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> or be in at least HOT STANDBY within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in COLD SHUTDOWN within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />".

Therefore, a TS violation occurred when a LCO action statement was not completed within the allowable time. The Shift Supervisor was notified and cell #9 voltage was measured at 1432 CST and found to be 2.20 volts. j D. CAUSE OF EVENT l

The causes of this event were:

1. Cognitive Personnel Error.

Although the voltage acceptance criteria identified in the procedure was not met, the foreman erroneously approved the measurements. This error was not the result of any unusual characteristics of the work location. I gg oau m. .u... m , ..._ m o.o = .

2. Procedure Inadequacy.

-Procedure 27915-C did not indicate that. cell voltage limits were tied to TS voltage parameter requirements nor did it indicate that a TS LC0 may need-to be entered based on measurements taken.

E. ' ANALYSIS OF EVENT Measurements taken both prior to the 11-22-89 event and again on 12-06-89 indicated acceptable = cell voltage values. Additionally, the other three battery banks in the system remained operable during the period of time involved.- Based on these considerations, there was no adverse effect on plant safety or public health and-safety as a result of this event.

F. CORRECTIVE ACTIONS 1.. The Georgia Power Company foreman involved has been' counseled regarding the importance of attention to detail in reviewing documentation.

2. A memo'has been sent to other appropriate personnel describing this event and the need for adequate reviews.
3. Procedure 27915-C will be reviewed and tevised as necessary by January 12, 1990.
