EVENT 98-1 CAUSE.
Scheduled Refueling Outage CORRECTIVE ACTION:
Guide Tube Support Pin Replacement.
J CRITICAL PATH WAS:
Completed S/G Eddy Current Testing on 2 & 3. Reloaded fuel, installed upper intemals and reinstalled reactor vessel head. Tensioned head and completed reassembly. Began heatup and mode ascension from mode 5 to mode 1. Performed appropriate functional testing in each mode, raised power after entering mode 1, and closed breaker.
EVENT 98-2 CAUSE.
Turbine Overspeed Trip Test CORRECTIVE ACTION:
Turbine test completed and unit retumed to service.
EVENT 98-3 CAUSE.
Repair ground in voltage regulator CORRECTIVE ACTION:
Ground repaired and unit retumed to service.
