SUPPLEMENTAL INFORMATIO_N FOR LICENSEE EVENT REPORT 82-02

1. Cause Description and Analysis On February 7, 1982, at 1805 hours0.0209 days <br />0.501 hours <br />0.00298 weeks <br />6.868025e-4 months <br />, with the unit at 81% power,llVH-1 breaker tripped on over-current during the performance of Periodic Test 8.1 (HVII Condensate Measuring System) and was declared inoperable.

This trip occurred immediately after HVH-1 was started. Initial investigation revealed that ilVII-l was rotating backwards which caused the over-current trip on unit startup. This reverse rotation was stopped, and IIVH-1 was started satisfactorily.

A thorough inspection of IITSI.-1-revealed a missing louver on the dis-charge damper, With IIVil-1 shutdown and the other HVII units opera-ting, this missing louver alloved air back flow through HVH-1 and caused the reverse rotation.

This event resulted in a degraded mode permitted by a Limiting Condition For Operation as defined by Technical Specification 3.3.2.2.a and is reportable pursuant to 6.9.2.b.2. The remaining HVH units were operating, and the containment spray pumps were demo;strated operable, so there was no threat to the public health and safety.

2. Immediate Corr,cctive Action In accordance with Technical Specification 3.3.2.2.a, the containment spray pumps were verified operable by performing Periodic Test 3.4A.

This test was completed at 2042 hours0.0236 days <br />0.567 hours <br />0.00338 weeks <br />7.76981e-4 months <br /> on February 7, 1982. As pre-viously stated, when the reverse rotation was stopped, llVH-1 was started satisfactorily.

A normal operating configuration for HVH-1 is running continuously.

Provided IIVH-1 is running, the missing louver does not present an operational (or safety) problem. In the event of a loss of onsite power,IIVH-1 is sequenced to start simultaneously with HVH-3 nnd prior to IIVH-2 and llVH-4. This would precluda reverse air flow through IIVH-1 from the other HVH units since they would not be operating.

Therefore, continued plant operation with IIVH-1 in its current con-figuration is not adverse to continued safe operation of the plant.

All licensed operators were informed of the special operational con-siderations concerning IIVH-1, and Operating Note 91 was issued to provide the required operator instructions. IIVH-1 was declared operable at 1637 hours0.0189 days <br />0.455 hours <br />0.00271 weeks <br />6.228785e-4 months <br /> on February 8, 1982.

3. Corrective Actions to Prevent Recurrence Since the occurrence of this event, the plant has been shutdown for refueling.. During this outage. all of the HVH units will~be thoroughly inspected, and all necessary repairs will be made prior to plant startup. No further action is considered necessary, tm
