SUPPLEMENTAL INFORMATION FOR l

LICENSEE EVENT REPORT 81-032

1. Cause Description and Analysis:

On December 5,1981, at 0850 hours0.00984 days <br />0.236 hours <br />0.00141 weeks <br />3.23425e-4 months <br /> with the unit at 64% power, "A" Boric Acid (BA) Transfer pump tripped during the recirculation of "A" BA Storage Tank for routine sampling. The pump was immediately declared inoperable.

Investiga tion of this event revealed that prior to 0554 hours0.00641 days <br />0.154 hours <br />9.160053e-4 weeks <br />2.10797e-4 months <br /> on December 5,1981, a transfer operation from the BA Batching Tank to "A" BA Storage Tank had been performed by the night operating shif t with the valve lineup in accordance with Operating Procedure OP-28-3-A. "A" BA Storage Tank was then placed in recirculation by the next shift crew. The Valve Lineup Procedure OP-28-3-A had recently been revised, was being used for the first time, and contained a typographical error in that it specified valve CVC-338

("A" BA Transfer Pump Suction) be " closed" instead of "open".

This error had no effect on the transfer operation; however, the

- error did result in no flow, pump overheating, and subsequent pump trip at 0850 hours0.00984 days <br />0.236 hours <br />0.00141 weeks <br />3.23425e-4 months <br /> during the recirculation operation performed by the next shift.

This event resulted in operation in a degraded mode permitted by a Limiting Condition as defined by Technical Specification 3.2.3.b which is reportable pursuant to 6.9.2.b.2. The redundant BA Transfer Pump was operable so there was no threat to the public health and safety.

2. Corrective Action:

"A" BA Transfer Pump motor was found to have an open winding as a result of the overheating. The motor was replaced and the pump was tested and declared operable at 1900 hours0.022 days <br />0.528 hours <br />0.00314 weeks <br />7.2295e-4 months <br /> on December 5,1981.

3. Corrective Action to Prevent Recurrence:

The defective procedure, OP-28-3-A, was revised on December 11, 1981, to include the correct position of valve CVC-338. The typographical error is believed to be an isolated case and no further action is considered necessary.
