SUPPLEMENTAL INFORMATION FOR LICENSEE EVENT REPORT 81-025

1. Cause Description and Analysis:

On November 2, 1981, a t 1906 hours0.0221 days <br />0.529 hours <br />0.00315 weeks <br />7.25233e-4 months <br />, "B" diesel genera tor '(DG) fa iled to manually start during the weekly Periodic Test of both DG's. "A" DG was demonstrated operable just prior to "B" DG's failure to start.

Two additional unsuccessful starts were attempted. "B" DG fuel oil lines were then primed with the hand pump, which resulted in successfully starting the DG, The failure of "B" DG to start is believed to be the result of debris in the ball check valve which allowed fuel to drain back to the day tank.

It should be noted that during a manual DG start (used during the Periodic Test) the air start solenoids will open for only 10 seconds which may not have provided adequate time to prime the fuel oil lines.

However, during an automa tic start of a DG (safeguards actuation), the time the air start solenoids are open is not limited, and therefore the diesel would probably have started on an automatic signal.

"B" DG's failure to sta rt, a degraded mode of operation as described in Technical Specification 3.7.2.d, is repot ted pursuant to 6.9.2.b.2.

2. Corrective Action:

The fuel oil lines and ball check valve were disassembled, inspected, reassembled and "B" DG returned to an operable condition at 2208 hours0.0256 days <br />0.613 hours <br />0.00365 weeks <br />8.40144e-4 months <br /> on November 2, 1981.

3. Co rrec t ive Ac t io n to Prevent Recurrence:

No further action is considered necessary.
