RNPD/88-0620 Enclosura to Sarielt P;ga 1 of 1 1987 ANNUAL REPORT - PRIMARY SAFETY AND RELIEF VALVE CHALLENCES 1.
EVENT:- Challenge to a Primary Relief Valve DATE: March 30, 1987 EVENT DESCRIPTION: Pressurizer Power Operated Relief Valve PCV-456 opened to relieve a. pressure transient in the Reactor Coolant System (RCS). The 30-Day Special Report issued for this event stated that the pressure transient was caused by a failure of the "B" Charging Pump Speed Controller, resulting in an imbalance between charging and letdown flows and that pressure Control Valve PCV-145 could not respond fast enough to control the flow imbalance. This resulted in PCV-456 opening to relieve the pressure transient (30-Day Special Report, RNPD/87-1670). However, based on later information, the cause was attributed to the "B" Charging Pump Fluid Drive. This event is still under investigation, and the results will be reported in a revision to the 30 Day Report 2.
EVENT: Challenge to a Primary Relief Valve DATE: June 9, 1987 EVENT DESCRIPTION:
Pressurizer Pows.r Operated Relief Valve PCV-456 actuated twice within seconds due to primary system pressure being allowed to increase to the setpoint on one of the pressure channels feeding the Low Temperature Overpressure Protection (LTOPP) System. The cause of the twent was attributed to the calibrated range of the pressure transmitters feeding the LTOPP system being too, wide a range to easily discern a prussure disagreement between the pressure channel instrumentation (30-Day Special Report, RNPD/87-3010). This condition is currently under evaluation to determine if a Plant modification is required to prevent recurrence of this event.

Carolina Power & Light Company (CP&L) hereby submits the 1987 Annual Report on Primary Safety and Relief Valve Challenges as required by Technical Specification 6.9.1.b.3.
On two separate occasions during 1987, the primary power operated relief valves (PORVs) were challenged. There was no challenge to a primary safety valve during 1987. Details on the challenges are provided by the enclosure.
