SUPPLDIENTAL INFORMATION FOR LICENSEE EVENT REPORT 81-026

1. Cause Description and Analysis On November 20, 1981, at about 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br />, with the unit at 55% power, a primary to secondary leak of approximately 1.5 gpm was identified in "A" Steam Generator, A plant shutdowr. was immediately commenced.

Inspection of "A" Steam Ge'.Krator showed that the leakage was the result of a leakinf, explosive plug, which had been installed in 1972.

The leak rate of 1.5 gpm exceeds Technical Specification 3.1.5.3 which is reportable pursuant to 6.9.2.a.3. All leakage was handled by normal plant systems so there was no threat to the public health and safety.

2. Corrective Action The plug was removed and a weld repair completed on November 27, 1981.
3. Corrective Action To Prevent Recurrence The above corrective action should be sufficient to preclude leakage from the effected tube in the future.
