3 86-012 CB PUMPXX This outage originated with the loss of both heater drain pumps and subsequent reactor trip from an overboration event. The outage duration was extended to allow repair of several leaking steam generator handhole covers discovered during an inspection immediately following the trip.

3 86-013 CH INSTRU While tierforming a maintenance surveillance test on the Steam Generator Pressure Instrumentation, the personnel involved failed to properly follow procedural instructions. This resulted in the loss of an active control channel, causing a reduction in feed flow to the effected steam generator. A steam generator low-level reactor trip occurred as level continued to fall.
