O'Reilly LER No.82-059 Page Two January 13, 1983 DETAILED DESCRIPTION OF EVENT On December 15, 1982, with the Plant in Mode 1, a detailed review of fire door surveillance procedures was being performed. It was discovered that some doors (see Attachment) requiring surveillance had inadvertantly been omitted from the latest revision of the Auxiliary Operator Logs. As these logs are used by the Auxiliary Operators to ensure that certain plant surveillances are performed, the subject doors were not checked.

Technical Spc+;fication Surveillance Requirement 4.7.10.2.(a) requires these doors to be verified closed once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

PROBABLE CONSEQUENCES The Plant had previously established a one (1) hour roving fire watch patrol. This roving one (1) hour fire watch patrol was in effect through December 15, 1982. Therefore, the chances that any fire cc. curring would have been detected in its early stages and would have been promptly brought under control.

CAUSE(S) 0F THE OCCURRENCE l During the last revision of the Auxiliary Operator Legs, the subject fire doors were inadvertantly omitted. Therefore, the doors were not checked.

IMMEDIATE CORRECTIVE ACTIONS TAKEN Upon completion of a temporary log, firewatch personnel checked the identified doors. The doors were all found to be in the closed position on December 16, 1982. These doors are verified

, and logged on a daily basis by firewatch personnel until the

! issuance of the revised Auxiliary Operator Logs.

. ACTION TAKEN TO PREVENT RECURRENCE

, A revision is underway to incorporate all Plant fire doors onto the Auxiliary Operator Logs. This will allow the verification that all fire doors are in the correct position once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> as per Technical Specification 4.7.10.2.(a). The Licensee plans to have this accomplished by January 21, 1983.
