Inspection Conducted February 13-16, 1990 (Report 50-445/90-06)

l Areas Inspected: Routine, unannounced inspection of receipt, storage, and bandling of emergency diesel generator (EDG) fuel oil and the station lubrication control program.

Results: Within the two areas inspected, no violations or deviations were identified.

It appears that the licensee has adequately reviewed and evaluated NRC Information Notice 87-04' issued on January 16, 1987, that was written as the result of the Arkansas Nuclear One (AN0) Unit 2 fuel oil (FO) starvation event which occurred on June 27, 1986. The licensee has incorporated an EDG fuel oil

~ program into their quality assurance program, and has a program in place to maintain adequate quality EDG fuel oil at CPSES.

2.. PROPER RECEIPT, STORAGE,- AND HANDLING 0F EMERGENCY DIESEL GENERATOR (EDG_)

The purpose of this. inspection was to determine if the licensee has a program to maintain adequate quality of EDG fuel oil that is stored.onsite, and to verify

that the: licensee has a program in place to purchase and ' store fuel oil-that meets TS: requirements.

2.1 Procedure Review The inspector reviewed selected procedures to verify that safety-related F0 activities are controlled and comply with Technical Specifications (TS).

The inspector also toured the general layout of the EDG F0 system and interviewed licensee personnel whom were directly responsible and knowledgeable of the EDG F0 program. ~The procedures reviewed are listed in the Attachment to this repor i

Review of:these procedures by the inspector indicated that the licensee has-developed written procedures to guide each routine F0 activity and that the procedures have sufficient detail to enable licensee personnel to perform the activity required.

-2.2 Program Review The inspector verified that the licensee has a program in place to purchase and store' fuel. oil that meets TS requirements and commitments.

This verification was done by interviews with personnel and review of procedures.

The quality of stored fuel oil is routinely determined at the point of

transfer from the truck to the EDG F0 storage tank and-again checked when it is transferred from the storage tank to the day tank.

Filters, strainers, and other components prone to fouling in the F0 system j

are routinely cleaned.

  • Stored fuel oil quality is determined by a sampling program such that

-

degradation resulting frem excessive water accumulation, oxidation, biological contamination, or other possible causes is detected.

In conclusion, it appears that the licensee has madequate program and l

procedures in place to preclude fuel starvation to the EDG.

3.

INSPECTION FOR VERIFICATION OF QUALITY ASSURANCE REQUIREMENTS REGARDING DIESEL GENERATOR FUEL OIL (TI 2515/93)

-The purpose of this inspection was to verify that the licensee has complied -

,

with 10 CFR Part 50,' Appendix B requirements regarding diesel generator fuel

-;

oil at CPSES.

To accomplish this task, the inspector interviewed QA. specialists, reviewed-procedures, reviewed maintenance records, and examined inspections / surveillance

records pertaining to EDG F0.

j One strengtn that was identified in this program is that QA under the Technical

. Specifications Assurance Program (TSAP) performs surveillances on all Technical

~

'

Specification (TS) line items at least once every 3 years.

This ensures that no TS line item relating to the EDG F0 is omitted. The procedures that were

' reviewed are listed in the Attachment to this report.

The inspector verified that the licensee has incorporated the diesel generator fuel oil into the quality assurance program and that the program appears to j

meet the 10 CFR Part 50, Appendix B Requirements.

4.

LtmRICATION PROG, RAM AND ITS CONTROLS

The purpose of this inspection was to determine if the licensee has a program in place to control the issuance and proper use of lubricants for permanent plant equipment.

4.1 Procedure Review

The inspector reviewed several procedures which established a program for the control of materials at CPSES. These procedures include controls for material requirements, material acquisition, storage, issuance, use, inventory, installation and maintenance, and disposal, i

The lubricant specification is'a design engineering specification which identifies approved lubricants for use in permanent plant equipment and

,

components and establishes the program for control of lubricants. This specification was originally a construction document but is being transposed from the Design Engineering Organization to the Operation Support Engineering, Technical Support, and Operations Chemistry Organizations.. Procedures reviewed are listed in the Attachment to this report. These procedures were considered to be satisfactory for their intended use.

'4.?. field'0bservations The inspector observed an electrical and mechanical maintenance crew sample and.

change oil lubricents for the motor and pump of both containment spray pumps in

the'"B" train. Discussions with the crews indicated that they were knowledgeable of their duties. Lubricants used were as specified by the job work order and the lubricant specification. Samples were taken as required by procedure. Ho discrepancies were observed.

4.3' Engineering Evaluations of Lubricants Used In early 1987, during a review of vendor technical manuals by the licensee, lubricant substitutions were identified on safety-related equipment that had no engineering)evaluationsor-documentations.87-063, " Unapproved Lubricants" was init Corrective Action Report (CAR engineering to perform an engineering evaluation of all the lubricants in use under the current operations lubrication list. Nonconformancereports(NCRs)

were. initiated where lubricants in use deviated from the acceptance criteria.

Review of several engineering evaluations by the inspector indicated that in most-cases the vendor-recomended lubricants were utilized except for the coupling lubrication. There were cases where unapproved coupling lubricant was installed in safety-related permanent plant equipment. This lubricant was Texaco Multifak EP2. Multifak EP2 was subsequently approved for most applications (those applications of 1800 RPM and below). NCRs were initiated where the use of Multifex EP2 represented a potential safety concern. As the result of this finding, Texaco lubricant 1912 was selected to replace all other coupling lubricants. The licensee provided adequate justification and test data to use Texaco 1912 on all couplings. The use of only one coupling

-6-lubricant _tends to avoid mixing of other lubricants and provides for better-control. The inspector verified that NCRs initiated because of a potential safety concern regarding the use of unapproved lubricants were addressed and corrective action had been taken.

The inspector's review also determined that the licensee presently has no preventive maintenance (PM) program for in place couplings.

Discussions with the licensee indicated that this issue was previousl was being tracked by the senior resident inspector (y identified by the NRC and SRI).

n 4.4' Conclusion Review of the lubrication program at CPSES indicates that the licensee has a program established to control the issuance and proper use of lubricants for permanent plant equipment. The use of only one-coupling lubricant is a

F strength in the program. Management's attention in this area was evident.

EXIT INTERVIEW

An exit meeting was held February-16,1990, with personnel indicated in

_ paragraph 1 of this report. At this meeting, the scope of the inspection and the findings were summarized. During this meeting the licensee informed the inspector that a PH program for couplings was being developed and should be in place by March 9,1990. The licensee did not identify as proprietary any of

the information provided to, or reviewed by, the inspector.
