Date Signed SUMMARY Inspection on October 1-4, 1979 Areas Inspected This routine, unannounced inspection involved 30 inspector-hours in the areas of licensee action on previous inspection findings, 10 CFR 50.55(e) items, 10 CFR Part 21 items, 10 CFR Part 21 procedures, IE bulletins, IE Circulars, construction activities, and ASME certification.
Results Of the eight areas inspected, no items of noncompliance or deviations were identified in seven areas; one item of noncompliance was found in one area (Infraction 395/79-30-09; 10 CFR Part 21 procedures. See paragraph 6).

Licensee Action on Previous Inspection Findings (Closed) Unresolved Item (395/79-08-01): Control and usage of cali-a.
brated test and measuring equipment. Daniel Procedures WP-VI-02, WP-VI-03, WP-VI-04, and WP-VI-05 have been revised to clarify re-quirements on control and usage of calibrated equipment. The inspector reviewed changes in tLe procedures. The inspector has no further questions in regard to control and usage of calibrated test and measuring equipment. This item is closed.

Inspector Follow-up Items (Closed) Inspector Follow-up Item (395/79-14-01): Engineering eval-a.
uatirn of deficient anchor bolt installation. The inspector reviewed the engineering evaluation of NCN No. 722 for support SWH-111. The licensee has determined that the support would have functioned satis-factorily without the one Hilti bolt wedged in place. The licensee and constructor are performing inspections of piping support and electrical supports to assure proper Hilti bolt installations. The problem of Hilti bolt installations has been reported to Region II as s 50.55(e) (see paragraph 8.C of this report). This item is closed.
b.
(Closed) Inspector Follow-up Item (395/79-26-01): Recording cause on SCE&G NCNS. The licensee has revised the nonconformance notice form to require a specific section for recording the cause of nonconfor-mances.
The inspector revieied three recent nonconformance notices (878C, 877E, and 876C) to arsure that cause is recorded on the non-conformance form. The inspector has no further questions on this matter. This item is closed.

Independent Inspection Effort a.
Compliance with 10 CFR 21 (1) General The purpose of this inspection was to ascertain whether SCE&G and appropriate responsible officers had established and implemented procedures and other instructions as required to ensure compli-ance with 10 CFR 21 requirements relative to the reporting of defects and noncompliances.
Inspector determinations are bated on the requirements of 10 CFR 21 as clarified by staff positions in NUREG-0302, Revision 1.
(2) Documents Reviewed (a) Nuclear Engineering Procedure No. NE-123, Rev. 5, Processing and Disposition of Potential Significant Deficiencies.
(b) Qualify Assurance Procedure No. 25, Reporting Unanticipated Occurrences to the NRC.

(3) Program Review and Implementation - SCE&G The inspectors reviewed the above noted controlling procedures.
The inspectors review of SCE&G procedures revealed the following problems:
(a) SCE&G has not developed and implemented appropriate proce-dures to assure the following:
deviations are evaluated (21.21(a)(1)); the Director or responsible officer is in-formed of all Part 21 affirmative evaluations (21.21(a)
(2)); the written report to the Commission contains the required information (21.21 (b)(3)); cnd the required records are maintained (21.51). The licensee, in lieu of the above requirements, had treated 10 CFR 21 in the same manner an 10 CFR 50.55 (e). Paragraph 5.6 of reference 6a(2) (a) permitted treatment of 10 CFR 21 noncompliances and defects in this manner and the licensee practiced this in the handling of noncompliances and defects. The inspec-tors informed SCE&G that this was an item of noncompliance.
This item is identified as an infraction 395/79-30-09, 10 CFR Part 21 procedures.
(b) SCE&G has not identified in procedures the responsible officer who is vested with executive authority over activi-ties subject to 10 CFR Part 21.
In discussions between the inspectors and SCE&G, SCE&G indicated that all officers are responsible officers.
The inspectors informed SCE&G that NRC requires that the responsible officer (s) be specifically delineated in procedures to assure compliance with 10 CFR Part 21 regulations. This aspect of the regulations will be reviewed again in conjunction with the licensees corrective action for the infraction identified in paragraph 6 a(3)(a).
b.
The inspector conducted a walk-through inspection of the reactor, auxiliary, and intermediate buildings; observed construction activi-ties, construction progress, and work practices, and talked to various craft and inspection personnel.

10 CFR Part 21 Reports a.
(Open) Item 395/79-30-01:
Containment airlock. On August 7, 1979, the licensee informed Region II of a problem with the leak tight integrity of Pittsburgh Des Moines Steel (PDM) manufactured contain-ment airlocks. PDM had reported this problem to Region I in accor-dance with 10 CFR Part 21.
Modifications to the airlock are scheduled for November 1979 according to the licensees letter of September 27, 1979.
This item will remain open pending modifications to the airlock and review by Region II.

(C.'osed) Item 395/79-30-02:
7300 series process I & C cards. On August 23, 1979, the licensee informed Region II of a problem with 7300 series process I&C cards supplied by Westinghouse. Westinghouse reported this problem to NRC as a 10 CFR Part 21 item. On September 27, 1979, the licensee reported that SCE&G does not possess any of the faulty process cards reported by Westinghouse. This item is closed.

a.
(0 pen) Item 395/79-30-03:
Incorrect cable splice plates. On August 31, 1979, the licensee informed Region II of deficiencies with regard to cable tray splice plates. Vertical splice plates were incorrectly substituted for horizontal p13tes due to the lack of horizontal plates on site. Corrective act. Mas are in progress to correct deficient splice plates. The licentees le'.ter, dated September 27, 1979, reported that deficiencies are totally documented by the site nonconformance control system, This item will remain open pending completion of licensee actions and review by Region II.
b.
(0 pen) Item 395/79-30-05: ASCO solenoid valves. On September 12, 1979, the licensee informed Region II of a problem with the use of an oil base lubricant in the installation of ASCO solenoid valves. The licensee's letter, dated September 27, 1979, reported that ethylene propylene elastomers in ASCO valves expand or swell in contact with oil, causing valve failure. This item will remain open pending further review by the licensee, issuance of a final report, and review by Region II.
c.
(0 pen) Item 395/79-30-05: Welded Hilti bolts.
On September 13, 1979, the licensee reported to Region II deficiencies in Hilti bolt instal-lations. Hilti bolts were found welded to support plates to correct installation problems. Reinspection programs are still in progress to identify all deficient installations. The licensees letter, dated September 27, 1979 reported that a final report will be provided upon completion of re-inspection and engineering evaluations. The item will remain open pending completion of licensee actions and review by Region II.
d.
(Open) Item 395/79-30-06: Feedwater check valve and pipe break. On September 13, 1979, the licensee informed Region II of possible problems with a pipe rupture in the feedwater line followed by a check valve slam. This situation would produce a large pressure peak that could damage the integrity of the piping system. The licensee's letter, dated September 27, 1979, reported valve manufacturer and architect engineer actions to correct this design problem. This item will remain open pending completion of the licensee's actions and review by Region II.

(Open) Item 395/79-30-07: Lightning arrestors. On September 7, 1979, the licensee informed Region II of a deficiency in 480 volt switch 1506 017

gear lightning arrestors. The licensee's letter, dated September 27, 1979, reported that low megger readings for lightning arrestors caused problems in the megger readings for 480 volt switch gear.
Lightning arrestors have been returned to the manufacturer and licensee actions to correct the problem have not been completed. This item will remain open pending completica of corrective actions and review by Regian II.

(Closed) Item 395/75-30-08:
480 volt motor termination taping.
On

September 18, 1979, the licensee informed Region II of a problem with the use of unqualified insulating tape. The cause of the problem was the use in safety related applications of tape intended for use on non-safety related applications. The unqualified tape has been removed from the field and action is being taken to replace deficient tape installations.

(Closed) Item 395/78-14-01:
Pipe whip rupture restraints. The licen-see submitted a final report to Region II on December 22, 1978.
Region II review of this item revealed an open nonconformance.
This nonconformance and a subsequent deficiency notice have been clesed.
Rework of the pipe whip rupture restraints is complete. Region II has no further questions on this matter. This item is closed.

Compliance with ASME Certification System Requirements a.
Effective ASME Code: The 1971 Edition of Section III of the ASME Code including Addenda through Summer 1973 is the Code in effect at V. C.
Summer.
b.
' Construction organization assuming the duties of a certificate holder:
Daniel Construction Company (DCC) has provided a construction organi-zation to assume the duties and responsibilities defined in Article NA-3000,Section III of the ASME Code. The DCC responsibilities and duties are described in the DCC Quality Assurance Manual.
c.
Valid Certificate of Authorization: DCC currently holds a valid Certificate of Authorization for the Code symbol NPT and NA.
SCE&G currently holds a valid Owner's Certificate of Authorization.
d.
Extension of Certificate of Authorization by ASME survey: DCC was issued an extension as a NPT, NA certificate holder on February 24, 1978. The NPT and NA Certificate of Authorization expires on February 24, 1981. SCE&G was issued on extension as an Owner certificate holder. The Owner's Certificate of Authorization expires on April 21, 1982.
e.
Activities of certificate holders include sections of NA-3000:
Actisities being performed by the NPT, NA certificate holder (DCC) are described in the DCC Quality Assurance Manual. Activities being performed by SCE&G as an Owner certificate holder are described in the SCE&G Quality Assurance Plan.

Status of Inspection and Enforcement Bulletins (IEB's)

(0 pen) IEB 79-15: Deep draft pumo deficiencies. On August 16, 1979, the licensee issued a letter in response to IEB 79-15.
This letter reported that V. C. Summer Nuclear Station has no deep draft pumps of the manufac-turer noted or used in the services described in IEB 79-15.
This item will remain open pending further review by IE Headquarters and Ret c; II into i
this matter.

Status of Inspection and Enforcement Circulars (IEC's)
The licensee has received, reviewed, evaluated, and is taking necessary actions on information contained in the following circulars:

IEC 79-17:
Contact problem in SB-12 switches on GE metalclad circuit breakers.
b.
IEC 79-18:
Proper installation of Target Rock safety-relief valves.
c.
IEC 79-19:
Loose locking devices on Ingersoll-Rand pump impe11ers.
The inspector discussed the above circulars with the licensee.
