Licensee Action on Previous Inspection Findings

(Closed) Unresolved Item 395/79-02-02:

Incorrect Cable Termina-tion. A review of the ideensee's startup test procedure SUM C-2,

" Electric Test Control Outline" and discussion with QA and startup test personnel indicated that the incorrect termination of safety related cable ESL 151B would have been corrected prior to equipment operation. This correction would have i.een made as a result of performing a verification of an electrical elementary wire diagram check which is required by SUM C-2.

In addition, pre-ops tests would also verify proper cable termination and circuit operability.

Startup Field Report (SFR) 354 documented that cable ESL 151B's termination, as designated on cable tenmination sheet and drawing D-210-188, was incorrect. This SFR, which was issued as a result of initial identification of the problem and before the verifica-tion of the electrical elementary wire diagram check and pre-ops tests, has cleared with correction of the cable termination. This

Insufficient Cable a.

Slack to Class IE Equipment. The licensee is using their noncon-formance rotice (NCN) 640E as the tracking document for this item.

An inspection has been made by the licensee's QC department on all applicable cable that had been terminated before January 8,1979.

This inspection was made by using a special computer printout

'

entitled " Selected Circuit Summary" dated January 8,1979. As a result of this inspection some of these cable were identified as having insufficient slack. Those have been or are to be replaced.

It presently appears that approximately 18 cables remain in this later category. These will have a final inspection and if necessary, will be replaced. Cables that met the seismic slack criteria have had QC documentation included in the cable records. Cables terminated after January 8, 1979 are inspected to new guidelines provided in Field Quality Control Procedure (FQCP) 7.1.2, " Cable Terminations" which includes inspection for seismic cable slack. This item is to remain open pending completion of work and/or close out of NCN 640E and future inspection.

(0 pen) Inspector Followup Item 395/78-30-02:

In Place Storage and Housekeeping. The current status and ongoing corrective action resulting from corrective action request (CAR) number 41 were examined. Revision 7 to Daniel Construction Company's procedure AP-VIII-03, " Cleaning, Handling, and Preservation", was issued.

This revision assigns responsibility to the discipline engineer to prepare a " Plant Equipment Maintenance Log (PEML)" for equipment.

The PEHL lists the requirements and periodic checks r maintenance protection for specified equipment. The mechanical superintendent is assigned responsibility for having the PEML periodic inspections and maintenance protection items performed. This item will remain open pending the closing of CAR 41 and evaluation on a future inspection.

Electrical (Cables and Terminations I) - Observation of Work and Work Activities and Review of Quality Records The inspector examined selected raceway supports, nondestructive tests and affiliated QC records for safety related cables EMC-84A, EMC-86A, EMC-87C, EMC-93B, EMC-96B,'EDE-29C, EDE-26A, EDE-24A, EDE-21A, CCM-17A, CCH-12A, CC-W-228, AH-C-77B, AH-C-116B, AH-C-67A, ES-V-17A, ES-L-162A, and ES-L-351A. personnel indicated that FQCP 7.1.6, "Installtion of Exposed Conduit and Conduit Supports" and FQCP 7.1.7, " Installation of Cable Trays, Supports,.

" are being revised. The revisions define that the

four inch open-wireway channels fall under separation criteria (i.e.

these systems are not to be considered closed conduit) and that the QC section is to maintain an official list of cable trays and four inch open-wireway channels that do not meet separation criteria. This list would be generated as a result of these system inspections and would be used in designating installation of barriers to satisfy the separation criteria. This item.will remain open pending licensee's final resolu-tion and evaluation of same on a future inspection of construction site activities.

(2)(Open) It - 395/79-09-02: Protection of Raceways. The QA department has requesteo comments on this item from several other departments affiliated with the construction activity. All comments had not been received in QA to formalize a final resolution. However, the inspector was informed that tentative plans were to have signs placed in various areas of the construction site. These signs would direct workers to keep off of the cable trays. This item will remain open pending licensee resolution and evaluation of same on a future inspection of

construction site activities.

Electrical (Components and Systems I) - Observation of Work and Work Activities and Review of Quality Records The inspector examined the Diesel Generator IB in the areas of handling, installation, records for protection and cleanliness after installation.

Procedures used in this examination were FQCP-3.2, " Storage, Handling and Issuing", FQCP-6.2.2, " Installation of Equipment", and AP-VIII-03,

"Cleanling, Handling and Preser'vation."

The calibration and trip settings for breakers in the 7.2 KV switchgear panel associated with bus XSWIDB were examined. The breakers examined were for the Component Cooling Pump Motors B and C, Charging / Safety Injection Pump Motors B and C, the 7.2 kv power supply breakers

identified by work packages numbers ES-61-007 and ES-61-008, and the supply breaker for 480 volt switchgear identified by work package number ES-10-019. The test startup manual was used in this examination.

Within the areas examined, no items of noncompliance were identified.
