4.

Unresolved Items Unresolved items were not identified during this inspection.

5.

Independent Inspection The inspector observed welding and welding related activities for two safety related piping velds. The code applicable to these welds is ASME Section III Class 2 (71S73). The welds were identified as:

Weld Stage of No.

ISOMETRIC System Completion FW-22 CS-88 Chemical Volume cotrol Fit up complete FW-6C1 EF-09 Emergency Feedwomet Welding in progress Both welds were examined for compliance with the Code. Rcquired documentation was also verified.

Safety Related Piping (Welding) - Review of Quality Assurance Implementing Procedures The inspector reviewed, in detail, safety related pipe welding quality assurance procedures of South Carolina Electric and Gas Company (SCE&G)

Daniel Construction Company (DCC), and Conam Inspection Division of Nuclear Energy Systems, Inc. (NES). These procedures were reviewed to determine whether appropriate and adequate procedures are included or referenced in the QA manual to assure that applicable welding, NDE and inspection activities are s trolled and performed according to NRC requirements and SAR commitments.

The applicable code for these activities is ASME Section III (71573).

Specific areas reviewed included the following:

Procedures that control or require the use of; qualified NDE techniques a.

in conformance with the applicable Codes, NDE procedures and persennel are qualified to applicable standards, contaminant free NDE materials.

DCC WE-I-02R2 DCC WE-I-05R4 NES 25-CRTP-002 R0 NES 25 CPTP-002-R1 b.

Procedures that contain provisions to assure that welding and NDE equipment is; proper type, within current calibration and that periodic checks made.

SC&EG FQCP 2.1.3 R9 DCC WE-I-02R2 (for densitometor calibration)

Inspection (QC) and/or work procedures which comply with the require-c.

ments of applicable codes for; evaluation of weld quality and evaluation of radiographic film quality.

Licensee Identified Items (50.55(e))

(open) Item 395/79-05-02: ASME Code Radiography. This item was reported to Region II on February 8,1979, as a 50.55(e) item. The licensee reported paper concerns, inadequate technique and potential relevant defects in ASME Section III radiography of pipe welds which had been previoucly acccpted by three radiographic interpreters. The inspector randomly selected six ASME Section III (71S73) welds in reattor coolant system piping and exaained tue radiographic film, techniques, and reports for compliance with Code require-

.ments.

These welds had been accepted in accordance with the licensee's 424 120

normal radiographic examination cycle. The licensee informed the inspector that radiography for all such accepted safety related welds is receiving an additional review as part of corrective action for the 50.55(e) item - as committed to in a letter to Region II dated March 12, 1979. The licensee identified five of the six welds selected by the inspector as having already received this review. The welds selected by the inspector are as follows:

Weld No.

Isometric Added Licensee Review W-1 RC-05 Yes W-9 RC-05 No W-11 RC-05 Yes W-1 RC-09 Yes W-1 RC-10 Yes W-2C1 R5 RC-10 Yes The inspector found questionable indications on film for welds W-9 and W-2C1R5. The inspector found that the licensee, in his added review, had also questioned the indications in W-2C1R5 and had requested a reshot.

The licensee has subsequently performed the added review on W-9 and informed the inspector that a reshot would also be performed on that veld.

This item remains open and will be subject to future reevattuation.
