Preservice Inspection - Review of Program the inspector reviewed licensee documents applicable to the preservice inspection program to determine conformance with FSAR commitments and regulatory requirements. Areas examined included responsibilities, training and qualification of QA personnel; audits; examination documentation; evaluation and acceptance requirements; control of deviations from estab-lished requirements; identification of the items to be examined; the methods and extent of coverage to be used for each item; establishment of inspection and work procedures; corrective action; document control; examination I394 217

All contractor (W-NSD) program documents for audits, responsibilities of QA personnel, control of devia-tions from established requirements, document control, equipment control, corrective action and records control were not required at the site and these areas could not be fully addressed in the inspection.

The licensee documents reviewed by the inspector for requirements applicable to the preservice inspection program were:

W-NSD Preservice Examination Program for Virgil C. Summer #1 with changes 1 and 2.

The inspector found that the licensee's program indicated that the inspec-tion would be performed in accordance with ASME Section XI (74S75). The program did not include steam generator tube eddy current baseline inspec-tion - the licensee's FSAR commitment with regard to Regulatory Guide 1.83 did not require this examination.

The inspector noted that the licensee's program did not include the pressure retaining bolting examinations added to the 74 edition of ASME Section XI with the Summer 75 addenda. These are item numbers B3.10, B4.12, B5.9 and B6.9 of ASME Section XI, Table IWB-2600. The licensee informed the inspector that the requirements would be checked to determine whether they were applicable to his plant. This will be re-examined in a later inspection and will be identified for record as Inspector Followup Item 395/79-27-01, some required preservice inspections missing from program.

Within the areas inspected no items of noncomplaince or deviations were identified.

Preservice Inspection - Review of Procedures The inspector reviewed licensee procedures pertaining to preservice inspec-tion for compliance with regulatory requirements and licensee commitments.

The code applicable to the licensee's inspection is identified in paragraph 5 above.

W-NSD Procedure ISI-11, Rev. 9, Am. 3, " Liquid Penetrant Examination Procedure" b.

W-NSD Procedure ISI-205, Rev. 2, Am. 3, "Manua1 Ultrasonic Examination of a Full Penetration Circumferential and Longitudinal Butt Welds" c.

W-NSD Procedure OPS-NSD-101, Rev. 5, Am. 2, "Preservice and Inservice Inspection Documentation" Procedures ISI-11 and -205 were reviewed for approval by authorized licensee personnel and the Level III examiner and for personnel qualification require-ments. Procedure ISI-11 was also reviewed for pre-examination surface preparation, drying time, penetrant application method, examination surface temperature, penetrant removal method, surface drying prior to developing, the developer and its application, examination for indications, and evalua-tion of indications. Procedure ISI-205 was reviewed for type of apparatus, extent of coverage, scanning technique, calibration requirements, sizes and frequencies of tranducers, beam angles, distance amplitude correction, use of transfer method, reference level for monitoring discontinuities, demon-stration of penetration, and levels for evaluation and recording of indications. OPS-NSD-101 was reviewed for recording and reporting requirements and acceptance limits.

Within the areas inspected no items of noncompliance or deviations were identified.

Preservice Inspection - Observation of Work and Work Activities The inspector observed preservice examination work and work activities for compliance with regulatory requirements and the licensee's commitments. The code requirements applicable to the licensee's preservice inspection are identified in paragraph 5 above. The licensee's program documents SCE&G Procedure AP-014 Rev. O and W-NSD "Preservice Inspection Program for Virgil C. Summer #1" (with Changes 1 and 2) were reviewed for identification of the proper number of examinations, examination methods, and extent of examinations. The qualifications and certifications of selected Level I, II and III NDE personnel were examined for identification of employer, person certified, activity qualified for, qualification level, certification period, approval signature, basis for certification, and annual visual acuity and color vision examination.

The inspector observed ultrasonic examination of pressurizer skirt weld U/T

  1. 15 which was performed in accordance with W-NSD procedure ISI-205 Rev. 2, Am. 3. The work was observed for availability and use of the approved procedure and equipment, NDE personnel knowledgeable in the examination method and use of the equipment, NDE personnel qualified for work performed, recording of required data, and performance of examination consistent with the applicable code and procedure. The inspector noted that while pressurizer welds were identified on a sketch there was no marking on i394 219

the vessel itself. The inspector questioned the adequacy of weld identifica-tion and location only through use of a sketch and asked that the licensee review this item. The inspector noted that quick identification of velds and weld location will be needed to minimize exposures in subsequent inservice inspections. This item will be Inspector Followup Item 395/79-27-02, Identification of welds for inservice inspection.
