Bryant(Ofection Chief, RCES Branch Date Sighed SUMMARY Inspection on July 31 - August 3, 1979 Areas Inspected This routine, unannounced inspection involved 27 inspector-hours onsite in the areas of licensee action on previous inspection findings, IE bulletins, IE circulars,10 CFR 50.55(e) items, and construction activities.
Results Of the five areas inspected, no apparent items of noncomplaince or deviations were identified.

Exit Interview The inspection scope and findings were summarized on August 3,1979 with those persons indicated in Paragraph 1 above.

Licensee Action on Previous Inspection Findings (Closed) Deficiency 395/79-18-02: Failure to follow procedures for cable tray support installation. The licensee provided a response to this noncm-pliance in a letter dated July 13, 1979. The licensee has taken action to correct the deficient electrical inspection forms cited in the noncompliance.
Also, action has been taken to review and audit all electrical inspection forms for expansion bolt installations. Deficiency Notice No. 2732E has been closed indicating that electrical inspection forms have been reviewed and corrected. An independent review of inspection forms will be performed by a clerk in the future to prevent recurrence of the problem.

Inspector Follow-up Items (0 pen) Inspector Follow-up Item (395/79-14-01): Engineering evaluation of deficient anchor bolt installation. Nonconformance notice no.
0722 has been written documenting a Hilti Kwik bolt installation where file pieces were wedged between the base plate hole and bolt to help achieve the required torque. The constructor has taken corrective action to correct the installa-tion and prevent recurrence of the problem. Additional inspections are planned to assure that similar conditions are not present in other installa-tions.
This item will remain open pending completion of the additional inspections, closecut of Noncenformance Notice No. 0722, and review by Region II.
6.. Independent Inspection Effort a.
The inspector conducted a walk-through inspection of the reactor, auxiliary, and intermediate buildings; observed construction activities, construction progress, and work practices; and talked to various craft personnel. Outside the reactor building the inspector observed the filling of the horizontal tendons with protective grease. The inspector also observed preparations for prestressing a horizontal tendon.
b.
The inspector selected three cable tray installations in order to determine if inspections had been completed per Field Quality Control Procedures (FQCP) 7.1.7 and 7.1.5.
The inspector reviewed documentation packets for the cable tray installations.
Inspection documentation was found to be complete in accordance with FQCP 7.1.7 and 7.1.5 requirements.
c.
The inspector accompanied the licensee and Daniel personnel on an inspection of recent pipe support welds. The licensee documented this inspection in surveillance report # 80019 dated August 3,1979. The inspection was performed to assess the welding inspection of safety related pipe supports.
Out of nine welds inspected, the licensee rejected one weld area of one support weld due to a " roll-over" condition on the weld. The licensee further concluded that welding and welding inspection of pipe supports has impraved since March 29, 1979. The licensee noted that borderline conditions existed in the area of weld undercut and slag.
The inspector also reviewed the visual inspection procedure criteria in QCP-II-06 for pipe hanger welds. The criteria state that undercut shall not encroach on the required section thickness. Discussions 1089 '92 _ __
~. . , -3-with the Daniel project quality manager indicated that clearer guidance on undercut could be provided for the weld inspection. The Daniel project quality manager indicated that action on clarifying weld undercut criteria would be taken. This item will remain open pending the Daniel clarification action on weld undercut and will be identified as inspector follow-up item 395/79-24-03: Weld undercut criteria for inspectors of pipe hanger welds.
Within the areas inspected, no items of noncompliance or deviations were identified.

Liceneee Identified Items (50.55(e)) a.
(0 pen) Item 395/79-24-01: Condensate storage tank found to be leaking.
A leak had developed in the tank bottom around the drain pipe. The licensee has issued Deficiency Report No. 889 to track correction of the deficiency. This item will remain open pending receipt of the licensee's final report, completion of corrective action, and review by Region II.
b.
(0 pen) Item 395/79-24-02: Deficiencies in pipe hanger welding. On July 30, 1979, the licensee informed Region II that deficient welds have been identified on safety related pipe hangers which Daniel QC inspectors had previously accepted. The pipe hanger welds have been rejected for deficiencies such as undersize weld, incomplete weld, nonfusion, cold lap, undercut, etc. On August 2, 1979, the licensee presented to the inspector a summary of the identified problems, actions taken on these problems, verification of actions taken, and continuing assessments of the problems. This was originally identified in a SCixG Corrective Action Request (047) in December 1978. Current plans are for a Daniel QC reinspection of all pipe hanger welding performed through June 1, 1979. This item will remain open pending receipt ot the licensee's final report, completion of corrective action, and review by Region II.
c.
(Closed) Item 395/79-05-01: Diesel generator exciter panel hold down bolts. A final report was submitted to Region II by the licensee in a letter dated December 22, 1978. This item had remained open pending the licensee review and closing of o Correction Action Request (CAR) to Colt and a Gilbert audit finding relative to stud installation for the exciter panels. Both the CAR to Colt and the Gilbert audit findiag have been closed by the licensee. The inspector has reviewed corrective action by Gilbert and Colt for this problem and has no further questions on this matter. This item is closed.
8.
Status of Inspection and Enforcement Bulletins (IEB's) (Closed) 79-Bu-ll: Faulty overcurrent trip device in circuit breakers for engineered safety systems. On July 12, 1979, the licensee reported in a letter to Region II that none of this type Festinghouse breakers are used in safety related systems. This item is closed.
