Dance, Section Chief, RONS Branch Date $igned SUMARY On July 7, 1980 a meeting was held with the licensee to discuss the results of their investigation and.the corrective actions they have taken concerning the failure of the High Pressure Coolant Injection (HPCI) and the Reactor Core Isolation Cooling (RCIC) systems to perform as designed following a reactor trip on June 26, 1980.
Georgia Power Company representatives gave a presentation which included: the identification of the June 26 system failures; the evalua-tion of maintenance, repairs and previous abnormal performance associated with the HPCI and RCIC systems; and the results of retesting the HPCI and RCIC systems.

Management Meeting A Georgia Power Company representative and a General Electric engineer gave a detailed review of their investigations and retest program. They concluded that the RCIC system malfunctioned due to a faulty relay or intermittent
limit switch. The HPCI and RCIC steam turbine controlswere also found to be out of balance with respects to smooth steam turbine operation. Signifi-cant adjustments and alignments were required in order to mitigate overspeed and high steam flow D/P turbine trips. Following the aforementioned mainte-nance and system calibration several HPIC and RCIC system functional tests were completed satisfactorily.
The licensee will be making changes to further improve the HPIC and RCIC systems reliability as follows: formal training in new system testing requirements will be given to instrument technicians and the reactor operators will be required to review the June 26 events; HPIC and RCIC surveillance

procedures will be revised where appropriate; a committee is being established to review HPCI and RCIC design, testing, operation and maintenance activities; The installation on a permanent or plug-in type basis transient monitoring equipment for all HPCI and.RCIC systems.

An NRC. representative acknowledged the licensee's presentation and indicated the need to provide the new information obtained as a result of these investigations to other Boiling Water Reactors (BWR's) with similar HPCI and RCIC systems.
A General Electric reprerentative indicated that the mecSnisms available would be used to notify and provide the information to other BWR's.
