On June 30, 2022, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Edwin I. Hatch. On July 25, 2022, the NRC inspectors discussed the results of this inspection with Johnny Weissinger and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Edwin I. Hatch, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. Refer to https://www.nrc.gov/reactors/operating/oversight.html for more information.

List of Findings and Violations

No findings or violations of more than minor significance were identified.

PLANT STATUS

Unit 1 began the inspection period at 100 percent rated thermal power (RTP). On May 23, 2022, the operators shut down the unit to perform maintenance on the F safety relief valve (SRV).

The unit was restarted on May 25, 2022 and reached approximately 100 percent RTP on May 31, 2022. The unit operated there for the remainder of the inspection period.

Unit 2 began the inspection period at 100 percent RTP. On May 29, 2022, the operators reduced power to 65 percent RTP for a rod pattern adjustment and returned the unit to 100 percent RTP the same day. The unit operated there for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

71111.01 - Adverse Weather Protection

Seasonal Extreme Weather Sample (IP Section 03.01) (1 Sample)

(1) The inspectors evaluated readiness for seasonal extreme weather conditions prior to the onset of Summer Readiness for the following systems:

1. Plant service water (PSW) on May 9, 2022.

2. Offsite and alternate alternating current (AC) power systems on May 11, 2022.

71111.04 - Equipment Alignment

Partial Walkdown Sample (IP Section 03.01) (4 Samples)

The inspectors evaluated system configurations during partial walkdowns of the following systems/trains:

(1) Unit 1 A emergency diesel generator (EDG) system following testing, using 34SO-R43-001-1, on April 5, 2022.
(2) Unit 1 PSW system during 1A PSW pump replacement on April 26, 2022.
(3) Unit 1 residual heat removal (RHR) system in a shutdown cooling alignment following reactor shutdown for the replacement of the F SRV pilot, using 34SO-E11-010-1, on May 24, 2022.
(4) Unit 2 reactor core isolation cooling (RCIC) system during the outage of the high pressure coolant injection (HPCI) system, using 34SO-E51-001-2, on May 31, 2022.

71111.05 - Fire Protection

Fire Area Walkdown and Inspection Sample (IP Section 03.01) (6 Samples)

The inspectors evaluated the implementation of the fire protection program by conducting a walkdown and performing a review to verify program compliance, equipment functionality, material condition, and operational readiness of the following fire areas:

(1) Unit 1 and 2 EDG building on April 8, 2022
(2) Unit 1 reactor building 158 and 164-foot elevations on April 27, 2022.
(3) Control building 130-foot elevation on April 15, 2022.
(4) Diverse and Flexible Coping Strategy (FLEX) storage building on April 21, 2022.
(5) Unit 1 turbine building on April 29, 2022.
(6) Unit 2 reactor building 130-foot elevation on May 17, 2022.

71111.06 - Flood Protection Measures

Inspection Activities - Internal Flooding (IP Section 03.01) (1 Sample)

The inspectors evaluated internal flooding mitigation protections in the:

(1) Safety related pull boxes PB1-AT and PB2-BU and evaluated cable submergence protection on May 12, 2022.

71111.11Q - Licensed Operator Requalification Program and Licensed Operator Performance

Licensed Operator Performance in the Actual Plant/Main Control Room (IP Section 03.01) (1 Sample)

(1) The inspectors observed and evaluated licensed operator performance in the control room during a plant shutdown, they observed power decrease by reducing reactor recirculation flow to minimum, inserting control rods to 20 percent RTP, followed by a main turbine trip, and the insertion of a manual reactor trip to complete the shutdown.

They then observed the operating crew establish a controlled cooldown rate within technical specification (TS) limits, for a Unit 1 maintenance outage, on May 22 and 23, 2022.

Licensed Operator Requalification Training/Examinations (IP Section 03.02) (1 Sample)

(1) The inspectors observed and evaluated License Operator Requalification Program scenario H-LT-AF-00114, CPE Design - Loss of CRD, RFPT Controller Malfunction, Loss of Iso-Phase Bus Cooling, Loss of the 2A Drywell Chiller, Torus Leak (Isolation),

Loss of Feed Water Heating, HPCI Steam Leak, Emergency Depressurization, on April 13, 2022.

71111.12 - Maintenance Effectiveness

Maintenance Effectiveness (IP Section 03.01) (2 Samples)

The inspectors evaluated the effectiveness of maintenance to ensure the following structures, systems, and components (SSCs) remain capable of performing their intended function:

(1) Unit 1 and 2 EDG fuel oil transfer system Y52 due to maintenance rule condition monitoring events being exceeded, on May 20, 2022.
(2) Unit 2 C EDG for being greater than 75-percent of the allowed unavailability time, resulting in the EDG being near 10CFR50.65 (a)(1) status, on May 27, 2022.

71111.13 - Maintenance Risk Assessments and Emergent Work Control

Risk Assessment and Management Sample (IP Section 03.01) (5 Samples)

The inspectors evaluated the accuracy and completeness of risk assessments for the following planned and emergent work activities to ensure configuration changes and appropriate work controls were addressed:

(1) Unit 1 elevated risk due to the surveillance of low voltage switchyard fixed water spray systems for the E startup auxiliary transformer, on May 12, 2022.
(2) Unit 1 elevated risk due to division 2 RHR outage, from May 17 through 19, 2022.
(3) Unit 1 elevated risk during unit shutdown to replace the F SRV pilot, on May 23, 2022.
(4) Unit 2 elevated risk due to the HPCI system maintenance outage, June 1, 2022.
(5) Unit 1 elevated risk due to division 2 core spray (CS) system outage, from June 28 through 30, 2022.

71111.15 - Operability Determinations and Functionality Assessments

The inspectors evaluated the licensee's justifications and actions associated with the following operability determinations and functionality assessments:

(1) Condition report (CR) 10874075. Unit 1 A PSW pump with a degraded seismic restraint.
(2) CR 10874078. Three
(3) out of 16 bolts for Unit 1 A PSW pump bowl found outside of torque specification.
(3) CR 10878586, Unit 1 A EDG battery charger not operating properly.
(4) CR 10880391. Unit 2 division 1 of residual heat removal service water (RHRSW)piping identified with a localized point below calculated minimum pipe wall thickness.
(5) CR 10857368. Unit 1 reactor building to outside differential pressure unexpected decrease caused by blocking open secondary containment doors.
(6) Correction action reports (CARs) 304318, 309195, 309199. Unit 1 A PSW pump motor excessive vibration.

71111.18 - Plant Modifications

The inspectors evaluated the following temporary or permanent modifications:

(1) Isolation of cable SGX708C03 (associated with Unit 2 main disconnect circuit), on both ends, to support troubleshooting and repair.
(2) Removal of Unit 2 PSW valve 2P41R606 internals to restore PSW flow to the hydrogen and isophase bus coolers.
