i APPENDIX A NOTICE OF VIOLATION l Public Service Company of Colorado Docket: 50-267

Fort'St. Vrain Nuclear Generating Station License: DPR-34 Platteville, Colorado During an NRC inspection conducted on January 26-30, 1987, two violations of NRC requirements were identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987),theviolationsarelistedbelow:

A. Paragraph (f) of 10 CFR 50.49 requires that qualification of each component must be based on testing or experience with identical equipment or with similar equipment with a supporting analysis to show that the equipment to be qualified is acceptable.

Paragraph (k) of 10 CFR 50.49 states that equipment previously required by the Commission to be qualified to the D0R Guidelines need not be requalified.

Paragraph 5.2.2 of the D0R Guidelines states that the type test should only be considered valid for equipment identical in design and material construction to the test specimen. Any deviations should be evaluated as part of the qualification documentation.

Contrary to the above, Public Service Company of Colorado (PSC) failed to adequately establish qualification of Rockbestos coaxial cable installed at Fort St. Vrain (FSV). PSC failed to establish similarity between the manufacturers first generation of production cable installed in the plant, and later production runs of cable which were type tested as documented in the qualification file (EQ binders).

During a subsequent NRC Region IV inspection on April 6-10 and 20-24, 1987, the NRC inspectors reviewed EQ binder EQ-FSV-79 and determined that the documentation for Rockbestos coaxial cable adequately establishes environmental qualification.

This is a Severity Level IV violation (Supplement I)(50-267/8710-01).

B. Paragraph (f) of 10 CFR 50.49 requires that qualification of each component must be based on testing or experience with identical equipment or with similar equipment with a supporting analysis to show that the equipment to be qualified is acceptable.

Paragraph (k) of 10 CFR 50.49 states that equipment previously required by the Commission to be qualified to " Interim Staff Position on Environmental Qualification of Safety-Related Electric Equipment," (NUREG-0588), need not be requalified.

Sections 2.1(1) and 2.1(2) of NUREG-0588 require: (1) that equipment qualified to Category I conform to the requirements in IEEE Std 323-1974, "IEEE Standard for Qualifying Class 1E Equipment for Nuclear Power Generating Stations;" and (2) that analysis must be accompanied by sufficient partial type test data to support the analytical assumptions and conclusions reached.

Contrary to the above, certain Raychem N-PKVPSC(stubdid notand type) adequately WCSF-N(establish qualification in-linetype) splice of-configurations installed in the FSV plant. The PSC documentation in the qualificationfile(EQ-FSV-15)fortheseinstalledspliceconfigurations were inadequate to support qualification by similarity to identical splices qualified by type test during laboratory testing. Specifically, the NRC inspectors, during the plant walkdown on January 28, 1987, observed splices installed in plant equipment where the splice shim sleeve extended  !

past the-sealing sleeve, and sealing between the sleeve and the cable insulation was in question.

During a subsequent NRC Region IV inspection on April 6-10 and 20-24, j 1987, the NRC inspectors reviewed EQ binder EQ-FSV-15 and the approved (

qualification documentation as a result of a new qualification test performed on the most limiting existing installed splice configurations at FSV. The NRC inspectors determined that binder EQ-FSV-15 for Raychem N-PKV (stub type) and WCSF-N (in-line type) splices, now adequately establish environmental qualification to include the most limiting splice i configurations installed at FSV.

Public Service Company of Colorado has taken sufficient corrective action to i demonstrate that full compliance to the 10 CFR 50.49 requirements has been  !'

achieved as verified during the NRC Region IV inspections ~of April 6-10 and 20-24, 1987, and therefore, no written response is required pursuant to the provisions of 10 CFR 2.201.
