APPENDIX A NOTICE OF VIOLATION Public Service Company Dacket: 50-267/86-06 of Colorado Fort St. Vrain Nuclear License: DPR-34 Generating Station During an NRC inspection conducted or February 1-28, 1986, two violations of NRC requirements were identified. 'she violations involved the failure to maintain records of a change and the failure to take prompt corrective action regarding a question raised as to the operability of safety-related equip-ment. ,In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violations are listed below:

A. Failure to Maintain Records of Changes 10 CFR Part 50.59 allows the licensee to make changes in the facility provided that the change does not involve an unreviewed safety question.

The licensee shall maintain records of changes to the_ facility, which-includes a written safety evaluation which provides the basis for the determination that the change did not involve an unreviewed safety question.

Contrary to the above, the licensee made a change to a instrument piping support in that a seismic hanger was replaced by a rope without main-

taining a record of a safety evaluation showing that this change did not involve an unreviewed safety question.

This is a Severity Level IV violation (Supplement I.D) (50-267/8606-01).

B. Failure to Take Prompt Corrective Action 10 CFR Part 50, Appendix B, Criterion XVI requires that measures be -

established to promptly identify and correct conditions adverse to l quality. The licensee's approved quality assurance plan, Section I B.5.16, implements this by requiring corrective action forms be originated l and responded to within 15 days.

l Contrary to the above, the replacement of a seismic' support with a rope 1 was identified by a licensee inspection in November 1985. On February 20 l and again on February 24, 1986, it was determined that no corrective l action request had been initiated ~ that the adverse: condition had not I

been corrected, nor had any corrective action been started.

This is a Severity Level IV violation (Supplement I.0) (50-267/8606-02).

_2 Pursuant to the provisions of 10 CFR 2.201, Public Service Company of Colorado is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in '

reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be.taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
