APPENDIX A NOTICE OF VIOLATION Public Service Company of Colorado Docket:
50-267 Fort St. Vrain Nuclear Generating Station License: DPR-34 During an NRC inspection conducted on August I through SepteTiber 10, 1988, a violation of NRC requirements was identified.
The violation involved a failure to follow procedures.
In accordance with the "General Statement of l
Polic and Procedure for NRC Enforcement Actions " 10 CFR Part 2 Appendix C l
(1988 the violation is listed below:
Failure to Follow Procedures Criterion V of Appendix B to 10 CFR Part 50 and the licensee's approved quality assurance program require that activities affecting quality be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances, and be accomplished in accordance with these instructions, procedures or drawings.
The licensee's Procedures P-2, Issue 14. "Equipment Clearances and Operation Deviations," and SMAP-19, Issue 7. "Processing Equipment Clearance and Operation Deviations," provide requirements.
l responsibilities, and steps for issuing and processing clearances.
Sections 4.1 and 3.7 respectively of the above procedures provide I
instructions for the modification of clearances.
Contrary to the above, on Septerber 7,1988, while placing Clearance No. 24662, Fuse F7 on Panel N9212 was pulled.
This fuse was not listed on Clearance No. 24662 and no modification of the clearance was made to include Fuse F7.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR Part 2.201, Public Service Corrpany of Colorado is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
