APPENDIX A NOTICE OF. VIOLATION Public Service Company of Colorado Docket:
50-267 Fort St. Vrain Operating License:
DPR-34 During an NRC inspection conducted on March 1-31, 1988, violations of NitC requirements were identified. The violation involved the failure to follow procedures and deficient procedures.
In accordance with the "General Matement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violations are listed below:
Technical Specification AC 7.4.a requires that written procedures shall be established, implemented, and maintained for activities including plant operations, surveillance testing, and maintenance.

Emercency Diesel Generator Valve Lineup Licensee Procedure IVL-92-04 Issue 1, "Integrated System Valve Lineup and Checklist for Emergency Diesel Generators" lists the correct valve lineup for the emergency diesel generators.

position which was not in accordance with the applicable procedure or

Deficient Procedure System Operating Procedure 45-01, Issue 21, "Fire Protection System," and Integrated Valve Lineup 45-01, Issue 1, "Integrated System Valve Lineup and Checklist for S0P 45-01" specify the valve lineups for the engine driven firewater pump for normal operation.
Contrary to the above, three valves (unnumbered in the cooling water line to the engine driven firewater pump are not identified on the plant valve lineup check lists.
This is a Severity Level IV violation.

f11shandling of Control Rods Licensee Nonconformance Report 86-218 in Item 1 of Guidelines states, "Avoid touching the control rod with uncovered hands to r'inimize the amount of foreign materials depositea on the surface."
Contrary to the above, on March 23, 1988, licensee personnel working in accordance with NCR 86-218 handled control rods with bare hands.

Pursuant to the provisions of 10 CFR 2.201, Public Service Company of Colorado is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shcwn, consideration will be given to extending the response time.
