APPENDIX A NOTICE OF VIOLATIO_N Public Service Company of Colorado Docket: 50-267 Fort St. Vrain Nuclear Generating Station License: DPR-34 During an NRC inspection conducted on August 3-7, 1987, a violation of NRC l

requirements was identified. The violation involved failure to follow l

procedure. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Criterion V of Appendix B to 10 CFR Part 50 states, in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings." The licensee's quality assurance program as documented in i FSAR, Appendix B, paragraph B.5.5.2, states, in part, "All safety-related j maintenance, inspection and surveillance activities are performed to procedures authorized under controls imposed by the FSV Administrative j Procedure Manual." Procedure G-9, issue 7, " Controlled Work Procedures," ;

Section 4.10, requires CWP closecut reviews by Technical Service Engineering, the Work Review Committee, Nuclear Licensing and Fuels, Site Construction, and Quality Assurance prior to forwarding of completed packages to the Records Center.

Contrary to the above, the results packages for CWPs 86-0131, 86-0132, 86-0133, and 86-0134 were filed in the record center prior to completion of closecut reviews by Technical Service Engineering, the Work Review Committee, Site Construction, and Quality Assurance.

This is a Severity Level IV violation. (Supplement I)(267/8720-01)

Pursuant to the provisions of 10 CFR 2.201, Public Service Company of Colorado is hereby required to submit a written statement or explanation to this Office within 30 days of the date of the letter transmitting this Notice. This reply, should include for each violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
