APPENDIX A NOTICE OF VIOLATION Public Service Company of Colorado Docket: 50-267 Fort St. Vrain Nuclear Generating Station License: DPR-34 During an NRC inspection conducted July 16 thcough August 31, 1989, a violation of NRC requirements was identified. The violation invDlved working longer hours without a rest than permitted by Technical Specif?catiors (TS). In accordance with Enforcement the "10 Actions," General Statement CFR Part of Policy 2, Appendix C 1989)(and Prote&re Policy),

(Luforcement for NRC the violation is listed below:

The licensee s TS AC 7.1.1.2.b.8.a states, in part, "An individual should not be permitted to work more than 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> in any 24-hour period."

The licensee's TS AC 7.1.1.2.b.9 states, in part, " Authorized deviations to the working hour guidelines shall be documented and available for review by the Nuclear Regulatory Commission."

Contrary to the above, on August 2,1989, the senior resident inspector entered the control room at approximately 4-4:30 a.m. and found four t-eactor engineers working on a safety-related test of the reactor core.

These engineers had been working since approximately 7 a.m. the previous day, and there was no documented authorization for these individuals to work beyond TS limits.

This is a Severity Level IV violation. (Supplement I.D) (267/8916-01)

Pursuant to the provisions of 10 CFR 2.201, Public Service Company of Colorado is hereby required to submit to this effice within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violaticos, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
